Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2503849 Primary Facility 2016-07-20 Routine/Partial Inspection No Violations Noted
2692093 Primary Facility 2018-02-02 Routine/Partial Inspection No Violations Noted Toby Dishong is onsite bailing the well. They tagged a bridge at 1980' and plan to clean out the well. Will determine once well is clear if the well will be produced or plugged. If it is to be plugged, paperwork will be sent in at that point. There is an open top steel tank for fluids and waste.
2700167 Primary Facility 2018-02-22 Routine/Partial Inspection No Violations Noted At the time of inspection, rig has fished a set of jars and tool head, and a 5' bit out of the well. This was at 1978'. The bridge below was then knocked out and was able to get to 2815', which should be TD. The well was not making any gas so the well will be plugged. NOI paperwork was going to be mailed today.
2703441 Primary Facility 2018-03-06 Routine/Partial Inspection No Violations Noted At the time of inspection, cement was attempted to be set from TD (2815') to 1840'. However, the next day cement was tagged at 2223'. Toby attempted to pull the 4 7/8" casing from the well, which is on a packer, but it would not budge. So it was decided to perf 2 holes, every 100', from 1940' up to 1140'. He then attempted to shoot the collar at 1053'. Casing was not free. He attempted again at 938', but casing was still not free. Casing was shot again today with wireline at 922' and came free. Rig was pulling casing while inspecting. N alternate method was given to me requesting to leave the bottom portion of the 4 7/8" casing in place. Verbal approval had been given prior.
2704899 Primary Facility 2018-03-13 Routine/Partial Inspection No Violations Noted At the time of inspection, HB cementing was onsite preparing to pump cement. A small lined pit was built from surrounding dirt to handle any flowback from the well. Very little fluid ever went into the pit. Most fluid was sent to an above ground open top tank. 3 separate cement plugs were pumped. A total of 273 sxs Type 1 were used, 15.6 ppg, 1.18 yield. This should be enough to go from a depth of 2220 up to 300', but top will be tagged tomorrow.
2709425 Primary Facility 2018-03-15 Routine/Partial Inspection No Violations Noted At the time of inspection, no activity was taking place. Rig has been laid down. The tank and totes have all been loaded on to a flat bed trailer. Pit has been filled back in and the liner was removed. There was very little fluid in the pit so it was easily sucked out prior to removal. Top of cement was tagged at 432'. Pea gravel was added to surface. I took a picture of the most recent cement report for inspection. Well was cut off and a red plate with the API was welded on to the top. It was buried underground. Reclamation for part of location is completed.
2739070 Primary Facility 2018-06-08 Administrative/File Review No Violations Noted
