2629059 Primary Facility 2017-08-23 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, a NOI to plug was sent in to begin on 8/21/2017. There was no activity taking place at the well, however, a rig is setting at the well. There is also a mini-excavator onsite which was used to create a small work area around the well. After speaking with plugging operator, this well will not be plugged until later in the year now and a new NOI will be submitted at that point in time.
2678973 Primary Facility 2018-01-04 Routine/Partial Inspection No Violations Noted This well is being cleaned out and will be plugged in the next couple of weeks. NOI is being sent in. Toby Deshong has been working on pulling 5 1/2" casing from the well. One joint remains that has a packer. Several attempts have been made to loosen the packer, but it will not budge. The joint of pipe has been splitting and is becoming harder to get ahold of. Packer is around 2000' in depth. Well would be worked on tomorrow, but for fear of getting tools stuck, attainable bottom would be granted. Alternate method to plug would be sent in describing what has been done so far and why we agreed to attainable bottom.

2684439 Primary Facility 2018-01-11 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, HB Cementing was onsite and had just finished the last cement plug of the day. From just below 2000', 3 plugs estimated to be 400' each were pumped into the well. Trucks had to pump from the main road, down a small hill to the well. The location is very small.

Toby Dishong was the plugging operator and he plans to tag the top of cement tomorrow or next week, depending on the weather. Tubing was being pulled out of well and excess water was flowing into an open top tank.

Top of cement was at 750', 50' higher than thought. Operator had hooked on to 7", but doesnt seem to have parted after being shot. He has wireline scheduled to shoot and knock collar around 700' over weekend or next week.

The culvert and ditch that was close to the site was cleaned up to have better flow, ensuring erosion won't affect the site until vegetation starts to grow. Top of cement was at 280' and 3 ton of pea gravel were used to get to surface. A steel plate was attached with permit number welded on below plow depth.

Operator stated that rig was broken in to and several items stolen.
