2675159 Primary Facility 2017-12-19 Routine/Partial Inspection No Violations Noted At the time of inspection, HAD Drilling Contractors was onsite using a DR-24 rotary rig rotating 24" conductor into the 3H well. The same size conductor was set on this well to a depth of 80'. Cuttings were being sent to an open top tank. Pad is still being worked on. There were several piles of gravel onsite to be spread out on the location. Some matting boards were sitting on the pad as well. There were 32' sound walls surrounding the entire pad. The permit signs and Emergency Response sign were posted at the entrance to the access road.
2678970 Primary Facility 2018-01-03 Drilling/Alteration No Violations Noted At the time of inspection, Sidewinder 62 was picking up BHA for intermediate section. BOP was tested this morning. Surface casing was cemented using 465 sxs (102 bbls) Class A, 15.6 ppg, 1.23 yield. 68 bbls freshwater was used to displace. Plug was bumped and floats held. 22 bbls cement returned to surface. A float shoe and float collar 441') were set along with 7 centralizers.

No oil, gas, or water has been noticed so far. Containment appeared to be in good condition and chemicals/fuel were contained.

2684440 Primary Facility 2018-01-11 Drilling/Alteration No Violations Noted At the time of inspection, C & J was onsite setting up to cement production casing. Safety meeting took place while onsite. Intermediate casing had been cemented using a lead of 160 sxs Class A (36 bbls), 15.2 ppg, 1.26 yield and tail of 503 sxs Class A (109 bbls), 15.6 ppg, 1.22 yield. Displacement was 139 bbls water. Plug landed and floats held. 30 bbls cement were returned to surface. A float shoe and float collar were used along with 23 centralizers. 500 bbls/hr brine water was encountered starting at 1460'.

Production casing was set to 11838' with a float shoe and collar and 232 centralizers. Containment looked good and cementing was going well while onsite.

The permit numbers for the wells are: 1H-019-22717, 2H- 019-22698, 3H-019-22718, 4H-019-22695 and the 8H-019-22699. Secondary containment setup onsite. Erosion and Sedimentation best management practices installed and maintained. Site stabilized with seed and mulch.

2779550 Primary Facility 2018-09-24 Routine/Partial Inspection No Violations Noted On 9/24/2018, WQS Brian MacQuarrie onsite. Five wells in production, 1H(019-22717), 2H(019-22698), 3H(019-22718), 4H(019-22695) and 8H(019-22700). Six 400 bbl tanks in stainless installed within steel containment structure. Erosion and Sedimentation BMPs installed and maintained. Vegetative growth well established.
2830319 Primary Facility 2019-01-17 Routine/Partial Inspection No Violations Noted On 1/17/2019, WQSS Scott Lux and WQS Brian MacQuarrie onsite. Snow covered ground during inspection. Site consists of five wells in production; 1H-019-22717, 2H-019-22698, 3H-019-22718, 4H-019-22695 and 8H-019-22699. Tank battery consists of six 400 bbl tanks installed within stainless steel containment structure. On 9/7/2018, operator submitted well site restoration extension request through e-submission. The most recent well drilled was the 3H. Drilling on this well was completed on 3/2/2018. On 9/24/2018, WQS Brian MacQuarrie conducted inspection and observed that site was stabilized and erosion and sedimentation best management practices were installed and maintained. Therefore, the well site restoration request is approved until 9/6/2020.
