The same size conductor was set on this well to a depth of 80'. Cuttings were being sent to an open top tank. Pad is still being worked on. There were several piles of gravel onsite to be spread out on the location. Some matting boards were sitting on the pad as well. There were 32' sound walls surrounding the entire pad. The permit signs and Emergency Response sign were posted at the entrance to the access road.

Surface casing was cemented using 490 sxs Class A (107 bbls), 3% CaCl2, 2% Gypsum, 15.6 ppg, 1.23 yield. Displacement was 69 bbls water. Plug was bumped and floats held. 21 bbls cement returned back to surface.

BOP was tested on 2/7. Plan is to cement intermediate casing this evening. Company man stated they hit "smelly water" at 1400'. Containment looked to be in good condition.

2697740 Primary Facility 2018-02-13 Drilling/Alteration No Violations Noted At the time of inspection, Sidewinder 62 was drilling 8 1/2" hole. 5 1/2" production casing was sitting on pipe racks. Containment appeared to be in good condition and chemicals were within it. Intermediate casing was cemented using a lead of 135 sxs Class A (30 bbls), 2% CaCl2, 15.2 ppg, 1.26 yield and tail of 500 sxs Class A (109 bbls), 2% Gypsum, 15.6 ppg, 1.22 yield. This was displaced with 140 bbls water and landed plug. Floats held and 35 bbls cement returned to surface.
2700236 Primary Facility 2018-02-22 Routine/Partial Inspection No Violations Noted At the time of inspection, Sidewinder 62 was drilling on the 3H well. Production casing on the 1H was cemented using a lead of 830 sxs Class A (217 bbls),14.4 ppg, 1.47 yield and tail of 1240 sxs Class A (429 bbls), 14.8 ppg, 1.94 yield. This was displaced with 339 bbls water and landed plug. Floats held. 80 bbls spacer returned to surface and top of cement was estimated to be at 470'.

Production casing was cemented using a lead of 885 sxs Class A (232 bbls), 14.4 ppg, 1.47 yield and tail of 1005 sxs Class A (347 bbls), 14.8 ppg, 1.94 yield. Displacement was 303 bbls water. Plug was bumped, floats held. 75 bbls spacer returned to surface so calculated top of cement was at 520'.

The permit numbers for the wells are: 1H-019-22717, 2H- 019-22698, 3H-019-22718, 4H-019-22695 and the 8H-019-22699. Secondary containment setup onsite. Erosion and Sedimentation best management practices installed and maintained. Site stabilized with seed and mulch.

Erosion and Sedimentation BMPs installed and maintained. Vegetative growth well established.
2830322 Primary Facility 2019-01-17 Routine/Partial Inspection No Violations Noted On 1/17/2019, WQSS Scott Lux and WQS Brian MacQuarrie onsite. Snow covered ground during inspection. Site consists of five wells in production; 1H-019-22717, 2H-019-22698, 3H-019-22718, 4H-019-22695 and 8H-019-22699. Tank battery consists of six 400 bbl tanks installed within stainless steel containment structure. On 9/7/2018, operator submitted well site restoration extension request through e-submission. The most recent well drilled was the 3H. Drilling on this well was completed on 3/2/2018. On 9/24/2018, WQS Brian MacQuarrie conducted inspection and observed that site was stabilized and erosion and sedimentation best management practices were installed and maintained. Therefore, the well site restoration request is approved until 9/6/2020.
