Well drilled and equipped for production, but does not appear to be in production. Drilling completed on 4/17/81. Annulus and production equipment rusty, recommend operator ensure structural integrity has not been compromised. Brine tank appears ok. No industrial waste issues noted. No accelerated erosion or sedimentation issues with site or access road. Site has been adequately and uniformly restored. Department unable to located recent production reports, recommend operator resubmit to remain in compliance with regulation as well as update department on status of well, as today it does not appear to be in production.
2367390 Primary Facility 2015-05-01 Routine/Complete Inspection No Violations Noted At time of inspection, well tag present and legible. Brine tank appears structurally sound. Site appears permanently stabilized. Department contacted Fairman Rep. Sandy Adamo on 5-7-15 by phone regarding depression with standing water near well head. Mr. Adamo felt the hole was an old ‘rat hole’ for placing bit when well was drilled. Department recommends filling hole to maintain site stability. Follow-up inspection will be conducted at a later date. Inspected with WQS S. Foulk.
2369610 Primary Facility 2014-05-14 Administrative/File Review In Compliance with Policy Administrative inspection due to a phone call from Fairman Rep. Sandy Adamo on 5-14-15. Mr. Adamo called in response to inspection record #2367390 regarding a 'depression with standing water near well head.' Mr. Adamo stated he was at the site and would have the hole filled to ensure continued site stabilization. Follow-up inspection will be conducted at a later date to evaluate compliance.
