No production/activity reports or requests for inactive status have been received by the department in the proceeding 12 months, the operator is in violation for not notifying the department of well status (abandonment) as well as failure to plug an abandoned well. Operator failed to achieve permanent stabilization after completion of earth disturbance activities as well as a failure to restore well site within 9 months of well completion date of 12/28/1986. Rusty brine tanks and other production related equipment are located onsite, but not attached to well and department does not have written record of any agreement that equipment shall remain in absence of producing well. Access road exhibits signs of accelerated erosion and sedimentation concerns, as runoff created a diversion along access road, as well as ponding water on well site.
