1838211 Primary Facility 2009-10-21 Routine/Complete Inspection No Violations Noted According to well records, permit was granted on 2/16/06 and well was drilled to 4013¿ on 5/10/09. WRAC report was submitted and approved on 8/3/06. Well head consisted of 8¿ pipe reduced to 4¿ reducer to 2¿ production line with 3 shut off valves. Production equipment consisted of separator (200 psi), 100 bbl brine tank (90 bbl fluid), and dual phase data logger (75 psi/0 psi). Last inspected on 8/11/09. At the time of inspection, no E&S issues and no IW issues
1843142 Primary Facility 2009-11-09 Routine/Complete Inspection No Violations Noted At the time of inspection, the well was properly tagged, but no in production. The well was adequately restored with 70% vegetative cover. No accelerated erosion or sedimentation issues were observed in relation to well site or access road. Brine tank was in sound condition. Noted that filter fabric fence had not been removed, since successful vegetative growth has been established it is recommended that the temporary E/S control be removed. Also note that placement of filter fabric fence in a culvert is not a recommend use for this BMP.
2109234 Primary Facility 2012-10-05 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, no E&S issues and no IW issues. Lance Cassidy was present at the site. North East set cement plugs at 3567'- 3001', 2710’-2250’, and 1250’- 1000' an d pulled tubing. The well was bailed dry and remaining well was filled with pea gravel.. A marker was going to be set with weld on permit # and vent.
