1946288 Primary Facility 2011-01-31 Routine/Complete Inspection No Violations Noted At the time of inspection, well sign and ES plan onsite. Well actively being drilled. Access road and pad snow covered. Pad adequately rocked. Observed soil pile at entrance of access road, recommend operator stabilizes with temporary bmp or seed/mulch in order to resist the potential for erosion. One below grade pit onsite properly lined with adequate freeboard. Temporary bmps improperly installed, but present. No accelerated ES issues observed at this inspection, but recommend that in the future filter fence should be installed below brush/grub line in order to resist the potential for accelerated erosion. It is also recommended that operator continue to closely monitor and maintain all temporary bmp¿s until final restoration is complete. Operator plans to encapsulate drill cuttings at minimum of 20in above seasonal high ground water table. Top-hole cuttings were to be discharged according to regs, but due to snow frozen ground operator is unable to do so.

Filter fence present on down gradient side of site. Pit covered with 20 mil plastic with security fence. No freeboard issue. PC contracted by NTS. PC currently waiting the 8 hrs time from cement plug down (11:39 am). Conductor pipe set at 13 3/8¿ to 74 ft (sanded). No mine shaft or void encountered. Second stringer set at 9 5/8¿ pipe to 413 ft. with 145 sks class A and 8 bbl return.

2376011 Primary Facility 2015-06-01 Follow-up Inspection Outstanding Violations - No Viols Req'd On-site inspection conducted as follow-up to inspection record 2353820 (enf ID 321123) for failure to install well tag in a legible, visible, and permanent manner and for failure to submit a well site restoration report. At time of inspection, well appears drilled and equipped for production.Drilling occurred 1/28/2011- 2/2/2011. Brine tank appears sound, located within earthen dike.Well tag present, legible.AR is developing ruts, therefore the Dept recommends the Opr continue to monitor and maintain the access road as necessary in order to prevent any potential pollution events from occurring.Well-tender appeared to utilize access road during the Dept’s inspt.As site is permanently stabilized with uniform and adequate vegetative growth, the Dept recommends the Opr ensure all temporary BMPs are removed as per current regulations.To date, the well site restoration report has not yet been received by the Dept.Viols issued under 3211(h) have been corrected. Outst viols exist under 78.65(3). Add'l comments on paper SAIR.
2376135 Primary Facility 2015-06-03 Administrative/File Review Outstanding Violations - No Viols Req'd Admin Inspection in relation to inspection conducted on 6-1-15 with WQS S. Foulk. Department called Operator Steve Phillips by phone on 6-3-15 and left a message. Mr. Phillips returned call the same day. Department recommended removal of temporary BMP’s; stated that access road was developing ruts; and that site restoration report was not submitted. Mr. Phillips responded that he would take care of all issues in a timely manner and would call when they were addressed. He remarked that he did not use the top section of the access road, but entered the site by the bottom road. Mr. Phillips inquired about well tag and Department responded that the well tag was present, legible and permanently affixed and is in compliance. Outstanding violations still exist for this site (reference Inspection #2353820). Department stated that a follow-up inspection would be conducted in approximately one week.
2383880 Primary Facility 2015-06-29 Administrative/File Review Outstanding Violations - No Viols Req'd Admin inspection in response to follow-up phone call on 6-29-15. Department contacted Operator by phone and recommended Operator remove and dispose of uninstalled temporary BMP’s on site and remove temporary BMP stakes. Department recommended maintaining access road. Operator stated he was waiting on a ‘mini-excavator’ to aid in removal of BMP’s and it should be on site soon. Department stated a follow-up inspection will be conducted in about 10 days. Previously issued violations are outstanding for this site.
2383913 Primary Facility 2015-06-17 Administrative/File Review Outstanding Violations - No Viols Req'd Admin inspection to follow-up phone to inspection #2376135. Department contacted Operator by phone on 6-17-15 and left message. Operator returned call later in day. Department stated a follow-up inspection would be conducted later in the day to assess compliance. Operator stated his workers would not be on site until 6-22-15. Department may wait until after 6-22-15 to follow-up. Previously issued violations for this site are still outstanding.
