Carrizo contracted Northeast Energy. 9 5/8” pipe was set at 1440 feet. With 724 sks of Type I cement with 17 bbls of return. Currently, depth of the well was at 8, 100 ft. Well was going to be logged and Union 48 rig was contracted to do the horizontal section.
2070121 Primary Facility 2012-04-17 Routine/Complete Inspection No Violations Noted At the time of inspection, no E&S issues were present. Carrizo contracted Northeast Energy. Conductor pipe (20”) pipe was set at 61 feet and was grouted in place. 13 3/8” pipe was set at 878 feet with 790 sks of Type I cement with 65 bbls return. Plug was down at 22:10 hrs at 4/16/12 by BJ. Currently, rig was under repair and setting up flow line. E&S control were present along access road and site pad.

2269202 Primary Facility 2014-05-06 Administrative/File Review In Compliance with Policy Admin Inspection as a result of a report of a spill. Email notification was dated 05/05/14 although it was not received until 05/06/14. Email stated cellar overflowed with rain water and water has sheen possibly from oilg/grease. Previous inspection on 09/19/14 (#2206278) states operator was "reminded to keep well cellar free from debris and fluids and mud". Follow up field investigation will be conducted by the Department and violations may be forthcoming pending further information gathering and inspection results.
2270954 Primary Facility 2014-05-07 Incident- Response to Accident or Event Violation(s) Noted FUI to admin inspt 2269202 for e-mail received by WQS Mark Millward from Opr rep Gary Byron on 5/5/2014 informing Dept that oil overflowed out of cellar onto pad & was discovered "late last week".At time of inspt, cellar filled to brim w/fluids.Black staining present on gravel adj to cellar & extends past surrounding cement blocks (also stained black) as evidence of previous overflow.No clean-up efforts appear to have commenced.Well drilled, not yet frac'd.WT present.TD date was 6/8/12.Per current regs, Opr shall restore well site w/in 9 mos of completion of drilling; hence, restoration deadline was 3/8/13.Site not yet restored.Dept unable to locate on-site PPC plan.Dept unable to locate ERP.Dept unable to locate on-site E&S plan.Viols being issued for failure to properly control & dispose of residual fluids, for failure to have an ERP on-site, for failure to have a PPC plan on-site, for failure to restore well site, & for failure to have an E&S plan on-site.Add'l viols may be forthcoming upon review of ESCGP-1 as not located on-site at this time.

2539428 Primary Facility 2016-11-14 Follow-up Inspection In Compliance with Policy Single well Marcellus site.

Follow up Inspection to Inspection Record #2270954. where violations issued for failure to control and dispose of residual fluids (corrected under Inspection Record#2467237), failure to have ERP on site, failure to have a PPC plan on site (corrected under Inspection Record #2292287), for failure to have E&S plan on site (corrected under Inspection Record#2292287), and for failure to restore within 9 months. At time of inspection Site ID sign, Master Containment Plan, Site Restoration Plan, PPC plan, Emergency Response Plan, ESCGP-1 and E&S inspection sheet all on site. Gate located at entrance to well pad unlocked at this time. Department observed an area of earth disturbance near sediment trap located to east of pad. It appears a 'road' was created through this area which is within the LOD. Department recommends Operator continue to monitor and maintain access road and all areas within the LOD to avoid any potential E&S issues. Site has not reached >70% uniform vegetative growth.
