2646743 Primary Facility 2017-10-06 Routine/Complete Inspection No Violations Noted Routine inspection of Samson Exploration's Pardee-Curtain A Pad and Freshwater Impoundment (FWI) on October 6, 2017. No one was onsite during my inspection. The two Marcellus wells (023-20124 and 023-20121) have recently been plugged. The well remain in the observation phase. No plugging certificate has been issued to date by the Department. Each well is labeled with the API, well permit number, and operator contact information. The produced fluids tanks and secondary man-made containment have been removed from the site.

The freshwater impoundment (FWI) has been mostly dewatered (see photo). I viewed the discharge area from the FWI. Discharging from the dewatering of the impoundment is located in a grassy flat area west of the impoundment. No E&S issues were noted. This site is on Samsons list to be fully restored this year.

2648613 Primary Facility 2017-10-13 Plugging(Includes Plugged/Mined Through) No Violations Noted Routine inspection of a well recently plugged. 2" candy cane vent installed on well casing. No gas detected at the vent or outside of vent. Casing outside outside of vent had standing water, no bubbling. No issues observed.

2663508 Primary Facility 2017-11-21 Routine/Complete Inspection No Violations Noted Routine inspection of Samson Exploration's Well Pad A on November 21, 2017. No one was onsite during my inspection. Several rolloffs and frac containers are staged near the entrance of the well pad. Delineated wetlands have been fenced off with orange fencing.

The freshwater impoundment and well pad have been restored and put back to approximate original contour. It appears that final grading needs to occur. The site has not been seeded or mulched. Please note Chapter 102.22 requires the site to be temporary stabilized upon temporary cessation of earth disturbance activities that will exceed 4 days. Compost sock remains in place. No E&S issues were observe during my inspection.

Both wells located onsite have been plugged. Each well is represented with a tagged well monument. All well production equipment has been removed from the site.

2695994 Primary Facility 2018-02-20 Plugging(Includes Plugged/Mined Through) Pending Routine inspection of a plugged well. The well has a 2" candy cane vent erected as a marker with well.

Well needs to be marked with permit number in accordance with 25 PA Code 78.97.

2721400 Primary Facility 2018-04-23 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of two unconventional wells, the C4 and C10, was conduced April 23, 2018 on this pad which is now being reclaimed. Both wells had a 2" candy cane vent pipe and sign erected as a plugged well monument. All casings are below ground level. There was no visual or audible flow from either vent. No obvious issues were observed.
2745095 Primary Facility 2018-06-22 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at Samson Explorations Pardee & Curtin Pad A on June 22, 2018. The well pad and FWI have both been restored. Everything is well vegetated and exceeds 70% uniform, perennial vegetative cover. Compost sock has been removed. No signs of E&S issues were observed. The site appears stable. I recommend submitting a Notice of Termination for this ESCGP along with the "As bulit" plans.

Both plugged wells are represented with a labeled well monument. I recommend submitting a post plugging restoration report for the plugged wells.

2760150 Primary Facility 2018-08-01 Routine/Complete Inspection No Violations Noted On behalf of Samson Exploration, ARM Group Inc. has submitted a notice of termination (NOT) for Pardee Curtin CO C-4 (Well Pad A) covered under ESX10-023-0004. This ESCGP covered both the construction and reclamation of Well Pad A and the Freshwater Impoundment (FWI). The Department received this NOT on July 16, 2018.

On August 1, 2018 Department representative Sue Herzing conducted a restoration inspection at the above listed site with Steve Trujilo of Samson Exploration and Joe Kunst of ARM Group. The well pad and FWI have both been restored back to approximate original contours. We walked throughout the restored FWI and restored well pad during the site inspection. Both the well pad and FWI meet 70% uniform, vegetative cover. All temporary E&S controls have been removed.

Two Marcellus wells have been plugged (023-20121 and 023-20124). Each well is represented by a tagged well monument. Post plugging site restoration reports have been submitted for the above listed wells.
