The 12H and 13H had no major issues observed during the inspection. The 2H and 3H were bubbling in the cellar (violations pending post-plugging evaluation). No violations were noted on the C5, which was bubbling last inspection turned out to be an open vent that was underwater for backside of production casing. I put a gauge on the vent; no pressure, less than a trace of flow. |

On August 1, 2018 Department representative Sue Herzing conducted a restoration inspection at the above listed site with Steve Trujilo of Samson Exploration and Joe Kunst of ARM Group. The well pad has been restored back to approximate original contours. All run-off from the site is being directed towards the wetland. We walked throughout the restored well pad during the site inspection. Overall the site exceeds 70% uniform, perennial, vegetative cover. All temporary E&S controls have been removed. Four Marcellus wells have been plugged (023-20134, 023-20135, 023-20185, and 023-20186). Each well is represented by a tagged well monument.
