2575294 Primary Facility 2017-03-27 Routine/Complete Inspection No Violations Noted Well stimulation work continues on the Rich Valley C09Dwell pad. The C09D well pad is occupied by (9) Marcellus wells and (1) Utica well. Stimulation work today is a zipper frac on (2) Marcellus wells with (3) additional Marcellus wells remaining to be stimulated. Water is supplied to the C09D location via a 16 inch HDPE above ground temporary waterline from the Clermont water management facility. Fuel for engines on location is supplied by (2) fuel trucks spotted on secondary containment and fuel deliveries several times each day to replenish those (2) fuel trucks. (2) HCl trailers spotted on containment on the location, for acid treatment of perforations at the beginning of each stage. Secondary containment for water storage, light plants, fuel trucks, chemical additives and waste fluids were free of water or fluids spills. Well laterals are being stimulated generally in 190' stages with perforations on 60° spacings. At the completion of each well stimulation a kill plug is set approximately 100' ab

2654253 Primary Facility 2017-10-26 Routine/Complete Inspection No Violations Noted Routine inspection of Seneca's CRV Pad C09-D on October 26, 2017. There are a total of 10 active wells located on this Marcellus well pad. Each well is labeled with the API and well permit number. Each well is connected to it own gas production unit (GPU) and any production water from the GPU is plumbed into 1 of 6 (400) BBL produced fluids tanks located onsite. There are a series of 20 frac tanks manifolded together along the Western border. These frac tanks are stationed on multiple layers of containment (see photo 2). Water within this secondary containment had a conductivity reading of 700 uS/cm. These frac tanks are being stationed onsite for a flowback that will occur in the near future from wells located on the CRV Pad C09-G. Water from the series of frac tanks will eventually be pumped to the Clermont Water Treatment Facility for reuse.

All PCSM BMPs have been installed. The site meets 70% uniform vegetation. A notice of termination (NOT) has not been approved for this well pad. I recommend submi

2721905 Primary Facility 2018-04-23 Routine/Complete Inspection No Violations Noted At the time of this inspection combustable gas was detected on the 9 5/8" x 5 1/2" annulus vent. No other combustable gas was detected.
2809152 Primary Facility 2018-11-29 Routine/Complete Inspection No Violations Noted Routine site inspection of Seneca Resources CRV Pad C09-D on November 29, 2018. There are a total of 10 producing Marcellus wells located on this well pad. Each well is labeled with the API and well permit number. Each well is connected to it own gas production unit (GPU) and any production water from the GPU is plumbed into 1 of 5 (400) BBL produced fluids tanks located onsite.

All PCSM BMPs have been installed. In previous inspections I have mentioned that the site meets 70% uniform vegetation, but today everything was snow and/or ice covered. A notice of termination (NOT) has not been approved for this ESCGP to date. I recommend submitting an NOT form next Spring/Summer 2019. The pad remains at full built-out of 235' x 550'.

No violations noted.

2825389 Primary Facility 2019-01-10 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. A digital gauge on the well head assembly was showing 469 psi (pounds per square inch). The annulus casings 20 x 13, the 13 x 9 and the 9 x 5 were vented to surface. Combustible gas , 100% LEL (lower explosion limit) and 59% Methane was detected on the 9 x 5 annulus vent at this inspection. No violations were noted at this inspection.
