Ran department down hole camera to identify depth of 3 inch and any holes in 3 inch tubing to cause gas bubbling up the annulus. There is a hole in the 3 " tubing @ 100 feet. S and T supply is the contractor for the CFA grant being used to plug this well.

S and T pumped 125 gallon of water dth to kill the well. Well was still gassing with fluid level @ 690'.

Plan is to pump 30 sks cement 8/28 am from td to 690', after reviewing several well logs in the area. Crew will return 6/29 tag the plug and complete a gas check. If more cement is needed they will pump more to bring plug up to 690. They will toh to 100 feet below 3 inch and set another 50' plug. 8/29 4 pm shooters will shoot off 3 inch @ 450' and s and t will toh 8/30 am. Camera run is scheduled for 8/30 pm to see where shallow gas is coming from and bottom if surface casing.

2770666 Primary Facility 2018-09-05 Plugging(Includes Plugged/Mined Through) No Violations Noted Ran downhole camera in well after well continued to vent gas. Fluid in the hole is at approximately 110'. No gas bubbles were seen at the fluid surface. We ran the camera lower in the hole and found multiple holes in the casing where the fluid is being drawn out of the well bore. No gas bubbles were seen anywhere in the fluid. Steve Lencer took a gas sample for possible isotopic analysis. Upon pulling the camera back up, multiple holes were seen in the casing above the fluid level. S&T tagged the cement plug at 385'. S&T plans to set an additional plug and load the hole with water so we can see where gas is entering the well bore.
2771411 Primary Facility 2018-09-06 Plugging(Includes Plugged/Mined Through) No Violations Noted Ran downhole camera into well. Lencer received a 100% methane reading in the casing. Water at 73', no bubbling noticed in water. Top of cement tagged at 100' .S&T loaded hole with water. Gas appears to be coming in to wellbore via a hole in the casing at approximately 45'. S&T filled well to approximately 60' with anti skid and pulled 80' of casing. Camera run showed crevasse at 45'. S&T cleaned out the well bore and loaded the hole with water to approximately 40'. No gas bubbles were seen. Hole was filled to approximately 35'. No gas bubbles were seen. Lencer checked for methane and received a 3% of LEL reading. S&T is going to set a rag packer on 1 1/2" at 30' so the well can vent and be monitored.
2771664 Primary Facility 2018-09-06 Plugging(Includes Plugged/Mined Through) No Violations Noted Rag packer set on 2" at 30'. 10 sacks of cement were set on the packer, to surface. The 2" will have a "T" installed at the top to enable gas monitoring.

S&T is currently readying equipment for demobilization.
