2740909 Primary Facility 2018-06-12 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at BKV Operating LLC Hegarty A well pad on June 12, 2018 with Enviornmental Consultant Kevin Brady.

There is one Marcellus well (033-27032) drilled and stimulated on this well pad. This well is currently in Regulatory Inactive Status and will expire on 4/22/19. Zero pressure was observed on the well head gauge. No production equipment is located onsite. There is no pipeline to this well pad. A well site restoration report has been submitted and approved on 10/13/16 for this above listed well permit.

The well pad remains the original footprint size of 430' x 330'. PSCMs have been installed per the PSCM drawings. A notice of termination was approved by the Department on 10/25/16 for coverage under ESX11-033-0012(01).

Overall, the site appears in good condition. PCSMs appear to be functioning as designed. The area surrounding the well pad is very well vegetated. No signs of E&S issues were observed.

Production casing pressure is 4550 PSI, backside pressure is 4550 PSI, annulus intermediate by production pressure is 0 PSI. All production equipment has been removed from the wellsite. Annulus vent (9-5/8" x 5-1/2") is open and vented at time of inspection. 0% combustible gas was detected at the annulus; 0% combustible gas was detected in the area immediately surrounding the well. Production casing pressure is 4500 PSI, backside pressure is 4500 PSI, annulus intermediate by production pressure is 0 PSI. All production equipment has been removed from the wellsite.
