2595986 Primary Facility 2017-05-18 Follow-up Inspection Outstanding Violations - Viols Req'd Follow up inspection of Range Resources Mohawk South Pad on May 18, 2017 with Randi Matthew, Zach Subra, and Jessica Ullom of Range Resources. Range Resources is in the process of flowing back Marcellus wells 035-21314 and 035-21313. On May 9, 2017 Range Resources reported a release of 3-4 BBL of flowback water outside of containment. A notice of violations (NOV) letter (353296) and inspection report (2592323) were sent to Range on May 11, 2017. These violations were assigned to well 035-21313. The Department is awaiting for Range Resources response letter for the NOV.

There are a total of 20 (350 BBL) upright silos located near the centre of the pad stationed on man-made secondary containment. This is where the flow back release occurred on May 9th. No fluids were observed outside of containment during my inspection, the trench was dry. Multiple silos can be filled at a time. There is approximately 2 inches of water in this bermed containment. Field screen reading for conductivity were out of range. I reco

2605433 Primary Facility 2017-06-16 Incident- Response to Accident or Event No Violations Noted On June 16, 2017 I conducted an incident inspection at Range Resources Mohawk South Pad with contractor Sonny of Deeter Farms. On June 13, 2017 Randi Matthew of Range Resources reported a small spill (10 gallons) to the Department. She reported that the spill occurred from a manifold that still contained some produced fluids. Allegedly the contract removed the end caps from the manifold and then hoisted the manifold up in the air to move it to another location on the pad. In doing so, water was released. When I arrived onsite, the gravel in the impacted area had already

been removed, placed in a roll off box, and delivered to the landfill. The spill occurred approximately right in the middle of the pad. During my inspection a puddle of water approximately 10 ft. wide and 20ft. long represented the impacted area (see photo 1). I collected a field screen reading in this puddle for specific conductance and got 400 uS/cm. Contractors on site were in the process of removing frac tanks onsite. There were a seri

2613172 Primary Facility 2017-07-05 Follow-up Inspection No Violations Noted Follow up inspection of Range Resources Mohawk South Pad on July 5, 2017. No one was onsite during my inspection. On May 9, 2017 a flowback release of 3-4 BBL of flowback fluids were released outside of secondary containment located near the center of the well pad. Violations were assigned to Marcellus well permit 035-21313. Violations for well 035-21313 remain outstanding.

The trench still remains present where the flowback release occurred, no water was observed in the trench during my inspection. It appears no other soil has been excavated where the flowback release occurred.

There are a series of 10 frac tanks in a row along the Western border of the pad stationed on man-made secondary containment. Each tank is empty. Standing water was observed in the frac tanks secondary containment; field screen reading of 182.6 uS/cm for conductivity. 2648408 Primary Facility 2017-10-10 Routine/Complete Inspection No Violations Noted Weather was 70° and sunny at time of inspection. DEP OGI Ulyana Trynovich was on location at time of inspection. 5 1/2 x 9 5/8 annulus vent was plumbed to a poly tank. Poly tank had 100% lel and 100% methane at time of inspection. 13 3/8 x 9 5/8 annulus vent was open at time of inspection. 0% combustible gas was detected. There was no 20 x 13 3/8 annulus vent observed at time of inspection.
2811462 Primary Facility 2018-12-04 Routine/Complete Inspection No Violations Noted On December 4, 2018, Department representative Brad Kline conducted a routine site inspection at Range Resources LLC. Mohawk South Unit Well Pad. No E&S issues were observed during the time of inspection. The site is stable and well vegetated meeting the 70% uniform, perennial, vegetative cover.

There are a total of 2 producing wells located on this well pad. All wells are tagged with API/well permit numbers. Cellars are filled in. There are 2 GPU’s and 4 steel vertical production fluids tanks onsite. Both GPUs and production fluid tanks were stationed in secondary containment.
