2712689 Primary Facility 2018-03-29 Complaint Inspection No Violations Noted Inspected location due to complaint and request from Millcreek EMA Director due to well leaking, found that well was situated in the middle of a juniper bush, well was shut-in and was not and has not provided the hotel any gas in sometime, National Fuel detected gas in storm drain and in several places throughout parking lot and next to hotel, talked to operator and it was agreed that the well would be plugged so this issue does not occur again in future, well was then killed with 1600 gallons of fresh water and a 10 foot 1 inch vent riser was installed.
2712699 Primary Facility 2018-03-30 Complaint Inspection No Violations Noted Inspected location with EMA Director, used Dept. issued Altair 5 and did not receive any readings, National Fuel was then called to confirm readings, contractor was on location removing flange to prepare for plugging operations which are scheduled to begin 4/2/18, water level was also checked and an additional 600 gallons was pumped into well, after that the flange and vent riser was put back on well, contractor will be checking well throughout the weekend. If well is not plugged according to schedule violations will be sent out to operator.
2712831 Primary Facility 2018-04-02 Complaint Inspection No Violations Noted Inspected location, caution tape and markers still surround excavated trench, riser still attached to well and well is still slightly venting to atmosphere, fluid level in well unknown at time of inspection, took gas readings with Dept. issued Altair 5, no readings of gas where detected at time of inspection from storm water drain or point of excavation, plugging crew is anticipated to arrive later this afternoon for setup and well be plugged tomorrow 4-3-2018, Dept. has filled out NOI and will give to contractor for signature and turn back into Dept, National Fuel has been onsite taking gas readings all weekend, EMA turned power back onto section of hotel Saturday morning.
2713758 Primary Facility 2018-04-03 Complaint Inspection No Violations Noted Inspected location, plugging crew plugging well, plugging crew used approx. 120 sacks of cement and cemented from TD to surface, a metal plate with API number has been put on well.
2714584 Primary Facility 2018-04-04 Routine/Partial Inspection No Violations Noted Inspected location, received phone call from plugging contractor and EMA director that gas readings where being detected at location, when arrived on location National Fuel and EMA where onsite taking readings and running vac truck to aspirate the parking lot where gas was being detected, readings were still being detected after running vac truck so new test holes were made and found the spot where the gas was most concentrated and National Fuel excavated another trench to see if a pipeline or old well could possibly be and nothing was found at that point National Fuel began pressure testing and metering there service lines and found a possible leak.
2715128 Primary Facility 2018-04-06 Routine/Partial Inspection No Violations Noted Inspected location, took several gas readings with Dept. issued model 60 through existing monitoring points and new holes, detected small amounts of gas but gas immediately dissipated. Cement in well has went down approx. 8" from surface, no bubbling within well. Also took several gas readings near well.
2724859 Primary Facility 2018-05-01 Routine/Partial Inspection No Violations Noted Inspected location, received phone call from National Fuel 4/30/18 at 1515 that they had picked up gas readings again in the parking lot, arrived on location at 1530 and National Fuel had the aspirator truck out sucking gas out from parking lot, after they were done readings were taken and no gas was picked up in meters, 5/1/18 arrived on location and took several readings with Altair 5 and picked up combustible gas readings ranging from 10-20 percent and no readings of methane were detected, called National Fuel and asked them to come out and take readings with there meter to confirm no methane readings, National Fuel arrived on scene and detected no methane readings, other readings that were detected were CO readings, no CO readings have been detected before.
2725695 Primary Facility 2018-05-03 Complaint Inspection No Violations Noted Inspected location, when arrived on location National Fuel was beginning to dig up there main line to begin pressure testing the line for leaks, first they had to dig up two points where they could tap in and run temporary service to the rest of there customers on Peninsula drive, departed location @ 1400 they were still working on issue, when i arrived i took readings with model 60 and received 8 percent in parking lot.

Probe hole northwest corner of excavation .4% gas. Probe hole southwest corner of excavation 0 all gases. Probe hole 18" down on north side of excavation 100% LEL for 2 pumps then nothing model 60. 1 ppm CO with Altair5 no other gases.

2735031 Primary Facility 2018-05-24 Complaint Inspection No Violations Noted Inspected location to take gas readings in various areas on property, took reading with Department issued Altair 5, no readings were detected at time of inspection.
