2775382 Primary Facility 2018-09-12 Routine/Complete Inspection Violation(s) Noted The Department had previously received an erosion complaint regarding the Hatalowich #2 well site. At that time the complainant also indicated the Hatalowich #4 had a culvert that was not properly maintained and was not conveying stream flow.

On 09/12/18 at approximately 0735 hours I conducted an inspection of the site. Stream flow was backed up on the upslope side of an 18" culvert under the access road. The backed up stream was ponding then flowing over the access road and along a fence line. Where the water flow met the fence an approximately 2' erosion channel was observed. It is highly recommended the culvert be maintained to return the flow to the stream bed and that the erosion caused by its diversion is repaired.

2801636 Primary Facility 2018-10-26 Follow-up Inspection No Violations Noted On 10/26/18 at approximately 1325 I conducted a follow up inspection of the site, due to outstanding violations. The culvert appeared to have been replaced. Rip rap had been placed in the stream channel and on the access road. A secondary stream channel was present that had been created from the culvert previously not properly conveying the stream. Excavated material had been placed on the stream bank parallel to the access road. A large accelerated erosion channel that was created when the culvert backed the stream up had not been repaired. It is recommended this issue be addressed. A piece of what appeared to be culvert pipe was observed in the field near the crossing. It is recommended the scrap pipe be removed. The violations remain outstanding.
2843220 Primary Facility 2019-02-14 Follow-up Inspection Violation(s) & Recurring Violations On 02/14/19 at approximately 1325 hours I conducted a follow up inspection at the site. Repairs to the culvert and accelerated erosion had recently been made. However, recent heavy rains and improper culvert maintenance caused the culvert to become covered in silt, causing the stream to back up and change course. Very little water was observed exiting the culvert pipe. The altered stream course had caused sediment, rip rap, and debris to be deposited on the nearby fence line. Extensive accelerated erosion was observed along the fence and between the fence and stream channel.
