2600397 Primary Facility 2017-06-02 Site Restoration No Violations Noted On Friday, June 2, 2017, I conducted a site restoration inspection at the Patterson Creek Well Site located in Center Township, Greene County. The inspection was conducted in response to an operator submittal of a Notice of Termination (NOT) for the ESCGP (ESX11-059-0055) in accordance with 25 Pa. Code Section 102.22(a).

The Act 9 sign was clearly legible at the site entrance and all required documentation was available on-site. The access road and the well pad are in good condition and the site is stabilized with vegetation. The landowner has assumed responsibility for several areas along the access road and the well pad which are being cultivated per the landowner's intentions.

The secondary containment structure around the production tanks appears to be in good condition and functioning as intended. The gas wells appear to be in good working order and are labelled as required in 58 Pa. C.S. Section 3211(h). 2733651 Primary Facility 2018-05-04 Routine/Complete Inspection No Violations Noted On Friday, May 4, 2018, I conducted a routine-complete inspection at the Patterson Creek Well Site located in Center Township, Greene County.

Gas well 591360 and the associated infrastructure appeared to be in good condition and functioning properly. The well was labelled.

No violations were identified during the inspection.
