2438689 Primary Facility 2016-01-05 Incident- Response to Accident or Event Violation(s) Noted On 1/5/2016, I performed an inspection of Vantage Energy James Rice well pad site. Present at time of inspection was John Bender, Jerry DeRosa, Greg Seibert (Vantage Energy), Noel Messing and Ken Sentgeorge (SE Tech). The site is located in Jackson Twp., Greene County. The inspection was performed as a response to a reported spill by the operator.

The operator reported a spill of 82 barrels of flow back water to secondary containment, with 10 barrels leaving containment and discharging to the AST pad and tank pad slopes on 1/5/16. The discharge of treated reuse water occurred during hydraulic fracturing operations on the James Rice 13H and 14H wells. The operator had Weavertown Environmetal Group on site to begin cleanup activities and removal of impacted material for disposal, with direction of SE Technologies.

2451489 Primary Facility 2016-02-02 Compliance Evaluation No Violations Noted On 2/2/2016, I performed an inspection of Vantage Energy's James Rice well pad site w/ Kevin Maskol (DEP). Present at time of inspection was John Bender, Jerry DeRosa (Vantage), Noel Missig, and Tim Ravatsky (SE). The site is located in Jackson Twp., Greene County. The inspection was a compliance evaluation inspection related to a reuse water on 1/5/2016.

The Act 9 and well permit signs were in place and legible.

The slopes of the well pad are well vegetated and stable.

The operator reported a spill of 82 barrels of flow back water to secondary containment, with 10 barrels leaving containment and discharging to the AST pad and tank pad slopes on 1/5/16. The operator has completed hydraulic fracturing operations at the site, and has begun dismantling the reuse water treatment system at the AST pad. The operator stated during the meeting that the AST will be dismantled within 10 days of the meeting to begin site characterization for the spill remediation.
