Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2523218 Primary Facility 2016-09-28 Preoperation Inspection No Violations Noted

On 09/28/16 at approximately 0800 hours I conducted a pre-op inspection of the EQT Rivers Unit. The Act 9 sign was in place. All required documents were present on the site.

The well pad had been completed and all BMPs installed. EQT was currently installing the freshwater impoundment and AST pad at the time of inspection. There were no signs of erosion and all the BMPs appeared to be in good condition.

Two red line changes were approved at the time of inspection. The changes were to the location of two pad sumps on the pad. The movement of the sumps does not change the drainage areas. There were no violations observed at the time of inspection.

2532531 Primary Facility 2016-10-28 Routine/Complete Inspection No Violations Noted Horizontal Marcellus, Set 26" conductor @ 40.
2553128 Primary Facility 2016-12-28 Routine/Complete Inspection No Violations Noted Horizontal Marcellus, Drilling on air @ 384' (TD). Runnig Water Protection Casing and cementing tonight (12/24/2016).
2553132 Primary Facility 2017-01-13 Routine/Complete Inspection No Violations Noted Horizontal Marcellus, Drilling on air @ 2473' (TD). Runnig Intermediate Casing and cementing (1/13/2017). Packer Placement 1220' - 1234'.

2561185 Primary Facility 2017-02-10 Routine/Complete Inspection No Violations Noted Horizontal Marcellus, Alpha Hunter # 8 finish top hole drilling (2/9/2017). Rig down waiting to move to next location.
2587033 Primary Facility 2017-01-13 Routine/Complete Inspection No Violations Noted Horizontal Marcellus, Alpha Hunter # 8 drilled to 3769 ' (1/13/2017).
2605707 Primary Facility 2017-06-12 Routine/Complete Inspection No Violations Noted Horizontal Marcellus, Fountain Quail # 5 set a cement plug from 3,769' back to 2,373' due to fault. Tagged cement @2,473' and was drilled out to 2,503'.
2630628 Primary Facility 2017-08-29 Routine/Complete Inspection No Violations Noted Horizontal Marcellus, Stack out Top Hole Rig moving back to own yard (8/29/2017).
2752197 Primary Facility 2018-07-16 Routine/Complete Inspection No Violations Noted The well is located in a well cellar covered by steel grating. The top hole casings have been installed. A pilot hole has been drilled. The well is capped.
2790771 Primary Facility 2018-10-16 Routine/Complete Inspection No Violations Noted One of a series of wells with all wellhead equipment under ground level in basement and covered by grate. No wells onsite are identified by API Number or well name. All but two cellars are filled with water restricting OGI ability to inspect. No bubbling noted on any flooded basement. 5 of 15 wells are listed as inactive, remainder listed as active. Due to restricted view of wellheads it is impossible to determine production capability of any well. Inspection of location showed no apparent violation.
2846633 Primary Facility 2019-02-21 Compliance Evaluation No Violations Noted On 12/29/19 EQT observed a reported 10-15 gallon brine spill of unknown source at the site. An NOV was issued and remediation was conducted.

On 02/21/19 at approximately 1100 hours I conducted a compliance evaluation inspection. The impacted sump had been excavated and replaced. The rip rap at the sump pipe outlet appeared to have been replaced, as it did not exhibit staining observed in the previous inspection. It appeared the spill had been remediated. There were no violations observed at the time of inspection.
