I spoke on the telephone with Mr. David Guidas from EQT Production Company concerning the matter and met him on-site later that morning to assess the situation.
There was no quantity provided in the notification that I had received, although Mr. Guidas and the company man had informed me that they had initially reported an estimated four (4) bbls of OBM had been released through the flare. They were not sure of the quantity released at the time of making notification to the Department due to the darkness of the early morning. Following additional investigation, EQT had determined that approximately three (3) to four (4) gallons of OBM was released through the flare. On 11/26/18 at approximately 1200 hours I conducted an incident response inspection. I spoke via phone with David Guidas who indicated 10 gallons of produced fluid was spilled when a line containing impaired water was removed from a frac tank. Due to teardown activities at the site the secondary containment had a hole that allowed the fluid to reach the ground surface. The spill was remediated by SPSI. The spill has been addressed in compliance with DEP spill policy. |
