2809943 Primary Facility 2018-11-26 Incident- Response to Accident or Event In Compliance with Policy On 11/22/18 at approximately 0952 David Guidas of EQT reported an impaired water spill on the Shipman well pad.

On 11/26/18 at approximately 1200 hours I conducted an incident response inspection. I spoke via phone with David Guidas who indicated 10 gallons of produced fluid was spilled when a line containing impaired water was removed from a frac tank. Due to teardown activities at the site the secondary containment had a hole that allowed the fluid to reach the ground surface. The spill was remediated by SPSI. The spill has been addressed in compliance with DEP spill policy.
