Department records indicate Carnegie Prod Co as the operator of this well. Department recommends operator update contact information to reflect current owner/operator. E and S plan on site. Access road appears seeded and mulched and vegetation beginning to grow. At time of inspection well appears to be plugged. Permit number affixed on erected monument in a permanent manner. Department recommends operator ensure correct permit number is displayed. Department observes areas lacking vegetation (photo #1-2) and 3 areas where it appears animals have been digging (photo #3). Department recorded elevated conductivity readings in the 3 areas where it appears animals activity has taken place. Department collected soil samples; one background and 1 from dug out area near monument (photo #4). Department contacted operator rep Vanessa Crayne by phone from site and left a message. Mrs. Crayne returned Department's call later that day

2789747 Primary Facility 2018-10-12 Administrative/File Review Violation(s) Noted ADMIN inspection. Department received and reviewed soil sample results collected on 8-28-18. Based on these results, it appears a release of brine/other fluids occurred.

Violations are being issued for operator failure to collect brine and other fluids and for operator disposal of residual waste inconsistent with or unauthorized by the rules and regulations of DEP. A follow up inspection will be conducted at a later date.

2791850 Primary Facility 2018-10-16 Follow-up Inspection Outstanding Violations - Viols Req'd FUI to inspection record #2789747.

Operator reported that on 10-1-18 remediation of a 9 square foot area near the erected monument was conducted (see email correspondence, photo #1-2). Operator reported that soil was excavated to a depth of 6 inches. At time of inspection it appears that the above mentioned excavation has been completed. At this time this area is wet and not vegetated (photo #3). Department requests operator submit documentation outlining the steps utilized to remediate this area. Department observes a small area to north of well monument that is lacking vegetation. It appears animal activity is taking place in this location (photo #4) and soil probe conductivity readings in this area are elevated from background readings. It appears that operator remediation efforts were not sufficient and did not remove all the impacted soil. Department recommends operator ensure all impacted soil is removed from site and disposed of in a manner approved by law. Department requests operator submit docum

No vegetation is present in this area at this time (photo #3). According to NOV response, operator states that this area was seeded and mulched on 10-1-18. Due to lack of vegetative growth, Department recommends operator reassess area to ensure all impacted soil has been removed. Violations issued on 10-12-18 remain outstanding. A follow up inspection will be conducted at a later date.
