2575227 Primary Facility 2017-03-22 Routine/Complete Inspection In Compliance with Policy At time of inspection no E&S issues observed on access road. Well appears equipped for production. Well tag present and legible.

Brine tank appears structurally sound however, piping leading to brine tank appears structurally compromised. This pipe has several compromised areas along its length including rust patches and a hole approximately 2"X5" (see attached photo). Department observed fluid dripping from this hole and 2 other locations along the pipe. Vegetation under pipe appears stressed. Department contacted Operator Rep Sandy Adamo by phone from site. Mr. Adamo stated he would send the well tender out to the site immediately. UPDATE 14:18: Operator followed up with Department by phone, left message. Department returned call. Mr. Adamo stated well tender visited site. Fluid dripping from pipe is freshwater that fills the brine tank because tank is not used. Mr. Adamo stated brine tank is being emptied today and the tank will be removed next week. Department requested Operator take conductivity readin

2601011 Primary Facility 2017-06-05 Routine/Partial Inspection No Violations Noted Routine Partial Inspection to Inspection ID #2575227 where Department observed fluid dripping from piping leading to brine tank.

At time of this inspection brine tank has been removed from site. No dead or stressed vegetation present on site. A follow up inspection will be conducted at a later date.
