2738748 Primary Facility 2018-06-07 Compliance Evaluation No Violations Noted Compliance Evaluation Inspection to address outstanding violation against well 7H (073-20493) for use of an open-top tank to collect/store production water (PA Code 78a.57a). Violation originally noted on inspection from 3/16/18 (ID# 2707046). Operator has installed a fitted cover correcting the violation.

Site restoration violations against the ESCGP permit are still open and will be addressed on a separate inspection when restoration violations are corrected.

Met with Hilcorp rep. on site at 1045 hrs to discuss spill from 1/1/19. On 1/1/19, a tree fell and broke the valve off of a double-walled 250 gal compressor coolant tank (photos 1 & 2), causing approx. 229 gal of coolant (50/50 mix of ethylene glycol and diethylene glycol) to spill to the ground surface (soil-cemented limestone well pad).

2827495 Primary Facility 2019-01-16 Follow-up Inspection Outstanding Violations - Viols Req'd Follow-up inspection to incident inspection of 1/2/19. Comments from 1/2/19 inspection are included below for reference.

Operator has cleaned up fallen tree and scraped up contaminated limestone from the well pad surface. Contaminated material has been removed in a roll-off for disposal. Operator is waiting on sample results before back-filling area. Outstanding violations; no new violations.
