There were no ARD representatives on location at time of inspection. Well had one annulus vent that was plumbed to surface at time of inspection. Well had digital pressure monitoring equipment attached to the production tubing, the pay line, the production casing and the annulus vent. The pay line digital gauge was the only one with a readout and it read 183 PSI at time of inspection. There was a 0-5k PSI analog pressure gauge on the production tubing that read approximately 250 PSI. There was a 0-5k PSI analog pressure gauge attached to the annulus vent that read approximately 0 psi. There was a 0-15k PSI gauge on the production casing that read approximately 0 psi at time of inspection. 0% combustible gas was detected anywhere around the well at time of inspection. No violations noted at time of inspection.
2760165 Primary Facility 2018-07-27 Routine/Complete Inspection No Violations Noted On July 27, 2018 Department representative Sue Herzing conducted a routine site inspection at COP Tract 685 Pad A. The site sign, emergency response plan, monthly tank maintenance log, ESCGP, and weekly E&S inspection log were onsite for review. No E&S issues were observed during the time of the inspection. PCSM infiltration berms have been installed per the PCSM drawings along the well pad and pipeline ROW. These berms consist of seeded compost sock. Vegetation on the socks is spotty but the socks are serving their purpose. The well pad remains its original footprint size.

There are 6 producing Marcellus wells located on this well pad. All wells are properly labeled. Well cellars are full of water and no bubbling was observed in any of the well cellars. There are 5 GPUs onsite and (4) 400 bbl. production water tanks located in secondary containment. Conductivity reading within the produced fluids tank was 92.8 uS/cm.
