2751008 Primary Facility 2018-07-09 Routine/Complete Inspection No Violations Noted Weather was 81° and sunny at time of inspection. There were no ARD representatives on location at time of inspection. Well was plumbed into production line but was shut in at time of inspection. Well had digital pressure monitoring equipment attached to the tubing, the production casing and the Intermediate x production casing annulus vent. The monitoring equipment did not have a readout. There were analog pressure gauges attached to the Pay line, production casing and the annulus vent. The 0-6k psi gauge on the pay line read approximately 1800 psi, the 0-6k psi gauge on the casing read approximately 2200 psi and the 0-5k gauge on the annulus vent read 0 psi at time of inspection. No violations were noted at time of inspection.
2790623 Primary Facility 2018-10-16 Routine/Complete Inspection No Violations Noted On October 17, 2018, Department representative Brad Kline conducted a routine site inspection at COP Tract 356 Pad I, monthly tank maintenance log, and weekly E&S inspection log were onsite for review. No E&S issues were observed during the time of inspection. The site is stable and well vegetated exceeding the 70% uniform, perennial, vegetative cover. Temporary BMPs still remain.

There are a total of 2 producing wells and 1 regulatory inactive status well located on this well pad. All wells are tagged with API/we’ll permit numbers. No bubbling within the cellars was observed. There are 3 GPU’s and 1 sand separator. Both GPUs and production fluid tanks were stationed in secondary containment.

No violations noted.
