2576334 Primary Facility 2017-03-29 Routine/Complete Inspection No Violations Noted Routine Inspection; 3/23.2017. I entered the site at 14:45. At the time of the inspection, the David C. Duncan A 4-6H (081-20531, 20302, 081-20530) wells were in production. Currently, permit 081-21565 remains active until 4/17/2017. The well was permittedin 2015 and drilling has not yet been initiated. Erosion was noted on the northeast cut slope going down onto the pad surface. ESX10-081 0017 (02) (major modification) was issued on 7/18/2016 and includes PCSM (Post Construction Stormwater Management) plansfor the current well pad surface. Stormwater is controlled through the use of infiltration berms. Currently, the PCSM has not been implemented. No Violations Noted.

2583277 Primary Facility 2017-04-13 Routine/Complete Inspection No Violations Noted Arrived @ 11:50. Weather: clear, light breeze. There were 3 producing wells in a linear arrayNo personnel were onsite at the time of my inspection. For all three wells, there was water in the cellars but no bubbling. The 9-5/8” x 5-1/2” vent was shut to a gauge. No other annular vents were visible. All pressures readings are digital.

David Duncan A 5H: Top gauge: 643 psi, 5-1/2”: 734 psi, 9-5/8” x 5-1/2”: 667 psi. David Duncan A 6H: Top gauge: 628 psi, 5-1/2” 799 psi, 9-5/8” x 5-1/2”: 0 psi, mechanical lift David Duncan A 4H: Top gauge: 610 psi, 5-1/2” 674 psi, 9-5/8” x 5-1/2”: -2 psi There was foam injection on the 4H and 6H. Left location @ 12:30. There were no violations at the time of my inspection.

2785014 Primary Facility 2018-10-05 Routine/Complete Inspection No Violations Noted Three wells in the production phase. Three GPUs and two separators. One double walled steel production water tank near separators. No leaks from equipment. No violations.
