I entered the location at 09:48 and began the inspection. At the time of the inspection, the Eugene P. Nelson A 3H (081-20526), 4H (081-20544), and 202H (081-21256) wells were in production. The well pad area and access road are all stabilized and no erosion was noted. The well site remains at its originally constructed size and has not been restored according to the PCSM (Post Construction Stormwater Management) plan in the ESCGP. The ESCGP expired on 8/16/2015. The revised restoration assessment indicates this site will be re-permitted sometime in 2016 and those revisions will be implemented sometime in 2017.

The Department considers a well site to be restored under Section 3216(c) of the 2012 Oil and Gas Act when the oil or gas owners or operators meet the following:

1. All permanent post construction stormwater control features as identified in the Post Construction Stormwater Management/Site Restoration Plan are in place.

2724151 Primary Facility 2018-04-26 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of the Eugene Nelson A 4H was conducted April 26, 2018. The purpose of the site visit is to confirm and inspect the plugging of this well after receiving a certificate of well plugging earlier this week. At the time of inspection; the well was a conductor only with a plate on top, cut below grade in a cellar with a 2" candy cane vent installed extending above ground level with a marker as a plugged well monument. No obvious issues were observed at the plugged well. The well was plugged 11/23/2014 according to the certificate of well plugging submitted.
