There were no ARD representatives on location at time of inspection. Well had one annulus vent that was plumbed to surface. Well had digital pressure monitoring equipment on the annulus vent, the production casing and the tubing that did not have a visible read out. There was a 0-10k PSI analog pressure gauge on the pay line that read 0 psi at time of inspection. There was a 0-10k PSI analog pressure gauge on the production tubing that read approximately 200 psi at time of inspection. There was also a 0-10k PSI analog gauge on the annulus vent that read approximately 0 PSI at time of inspection. 0% combustible gas was detected in any area around the well at time of inspection. No violations were noted at time of inspection.
2760177 Primary Facility 2018-07-27 Routine/Complete Inspection No Violations Noted On July 27, 2018 Department representative Sue Herzing conducted a routine site inspection at COP Tract 685 Pad B. The site sign, emergency response plan, monthly tank maintenance log, ESCGP, and weekly E&S inspection log were onsite for review. No E&S issues were observed during the time of the inspection. PCSM infiltration berms have been installed per the PCSM drawings. These berms consist of seeded compost sock. Vegetation on the socks is spotty but the socks are serving their purpose. The well pad remains its original footprint size. Overall, the site is stable and well vegetated exceeding 70% uniform, perennial, vegetative cover.

There are 5 producing Marcellus wells and 1 plugged well located on this well pad. All wells are properly labeled. Well cellars are full of water and no bubbling was observed in any of the well cellars. There are 5 GPUs onsite and (4) 400 bbl. production water tanks located in secondary containment. Conductivity reading within the produced fluids tank was 58.4 uS/cm.
