I entered the site at 10:02. At the time of the inspection, the Jack Hipple A 1H (081-20472), 2H (081-20471), 3H (081-20470), 4H (081-20469), and 6H (081-20831) were in production while the monument of the plugged 5H (081-20468) was visible. ESX10-081-0069 expired on 9/1/2015 without being terminated. Currently, the site remains at the originally permitted size, has not been restored or reduced, and no PCSM (Post Construction Stormwater Management) has been implemented on the site.

The most recent update to the well site restoration assessment Anadarko provided on 12/22/2016 indicates infiltration testing was completed in 2016 and the site is planned to be permitted with an ESCGP-2 in 2017. At this time, the Department does not have a permit or permit application for the site. I recommend re-permitting the site (as planned) to comply with PCSM requirements and implementing that plan including well site restoration. No Violations Noted.
