2482678 Primary Facility 2016-05-06 Routine/Complete Inspection No Violations Noted I entered the location at 09:02 and began the inspection. At the time of the inspection, the John Good 1H (081-20955), 2H (081-20956), 3H (081-20957), 4H (081-20958), 5H (081-20959), and 6H (081-20960) were in production. The well pad area and access road are all stabilized and no erosion was noted. The well site has not been restored but the rock infiltration sumps have been installed based on the PCSM (Post Construction Stormwater Management) plan in the Major Modification approved 12/2/2011. The revised restoration assessment acknowledges the well site has not been restored and drilling ended on 5/30/2013. The well pad remains at its original size and no well permits have been issued since 2012.

The Department considers a well site to be restored under Section 3216(c) of the 2012 Oil and Gas Act when the oil or gas owners or operators meet the following:

1. All permanent post construction stormwater control features as identified in the Post Construction Stormwater Management/Site Restoration Pla

No annulus vent observed at the time of inspection. 0% combustible gas detected at the time of inspection. A smell associated with natural gas wells was detected, but could not determine the source. Well head digital gauge shows 324 PSI production tubing, and 573 on production casing.

2681195 Primary Facility 2018-01-03 Routine/Complete Inspection No Violations Noted The access road from the Flicks Run well pad to the well pad itself was in good condition and well maintained. A few inches of snow was covering the entire site. On the well pad, six wells were in production. Sand separators remain installed at the production units and both (for each well) are in containment. No staining was visible around any of the production equipment. Methanol was stored (covered and in containment) near the wellheads for injection.

The ESCGP permit has expired and Post Construction Stormwater Management (PCSM) has not yet been permitted or implemented. I recommend re-permitting the site and following the COA which indicates this site will be restored in 2019. No violations noted.
