2605610 Primary Facility 2017-06-05 Site Restoration No Violations Noted On June 5, 2017 I conducted a restoration inspection of Pennsylvania General Energy Company LLC (PGE) DCNR Tract 293 Pad B. Multiple folks where onsite working on the well heads. PGE submitted a notice of termination (NOT) to the Department for this pad and access road which is covered under ESX12-081-0027(01). The Department received the NOT on May 19, 2017.

The Marcellus pad remains its original footprint size of 700' x 651'. In addition, the access road is 1,176 ft. long by 94 ft. wide. Two PCSM bioretenion basins remain in place. The area surrounding the pad is well vegetated and meets 70% perennial vegetative cover or is stable with rock.

There are a total of 9 producing wells onsite. Well site restoration reports have been submitted for these 9 producing wells and have been approved. Each individual well is connected to its own sand separator and production unit. Each separator is plumbed to 2 steel tanks. There is an overflow from each tank to 4 poly production fluids tanks. 2703164 Primary Facility 2018-02-21 Routine/Complete Inspection No Violations Noted Routine inspection; 2/21/2018.

Ben Gamble (DCNR) and I entered the site and looked at the PCSM on the fill side of the well site. The basins were well-vegetated and the ESCGP was terminated in 2017. Water was leaving the basins through an under-drain system which subsequently flowed northwest, scouring a channel along the ESCGP permit boundary where perimeter E&S controls had been placed. We were able to hear water flowing through the basin, which suggested the detention time in the basin was quite low. Based on our observations, I recommend coordinating with the Department and DCNR to implement solutions which effectively handle stormwater and minimize concentrated flow from the site. No violations noted.

2822584 Primary Facility 2019-01-03 Routine/Complete Inspection No Violations Noted On January 3, 2019, Department representative Brad Kline conducted a routine site inspection at PGE Company LLC. DCNR Tract 293 Pad B. No E&S issues were observed during the time of inspection. The site is stable and well vegetated meeting the 70% uniform, perennial, vegetative cover. The pad has not been restored.

There are a total of 9 producing wells located on this well pad. All wells are tagged with API/well permit numbers. No bubbling within the cellars was observed. There are 9 sand separators and 9 line heaters. 2 steel and 4 poly production fluids tanks onsite. Line heaters and production fluid tanks were stationed in secondary containment. 2 reboilers are stationed on the pad.
