2691292 Primary Facility 2018-01-24 Routine/Complete Inspection No Violations Noted Weather was 26° cloudy and windy at time of inspection. Well has a digital gauge above the swab valve on the production tubing, on the 5 1/2 casing and on the 9 5/8 x 5 1/2 casing annulus. The tubing pressure was 404 psi, the casing pressure was 534 psi and the annulus pressure was 1 psi at time of inspection. Cellar was frozen solid at time of inspection. A second annulus vent was plumbed to surface but was closed and had no gauge on it for observation.
2761283 Primary Facility 2018-08-07 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at ARD Operating LLC COP Tract 357 Pad B on August 7, 2018. The site sign, emergency response plan, monthly tank maintenance log, ESCGP, and weekly E&S inspection log were onsite for review. No E&S issues were observed during the time of the inspection. The well pad was constructed on relatively flat ground. PCSM infiltration berms have been installed per the PCSM drawings. These berms consist of seeded compost sock. Vegetation on the socks is spotty but the socks are serving their purpose. In addition, a compost sock infiltration berm has been installed along the Western edge of the well pad. The well pad remains its original footprint size. Overall, the site is stable and well vegetated exceeding 70% uniform, perennial, vegetative cover.

There are 6 producing Marcellus wells located on this well pad. All wells are properly labeled. Well cellars are full of water and no bubbling was observed in any of the well cellars. There are 6 G
