2236468 Primary Facility 2014-01-09 Routine/Partial Inspection No Violations Noted Dallas Morris 25 onsite waiting for conductor to be drilled. No Vio.
2274688 Primary Facility 2014-05-21 Complaint Inspection No Violations Noted On 5/21/2014 Ms. Renner and I inspected the site. Severe erosion was noted on the cut slope of the infiltration basin and at the inlet swale entering the basin. In addition, erosion was noted in the roadside swale above the basin. Based on the erosion I recommend re-evaluating the basin design itself in addition to possibly adding additional cross culverts to reduce the volume of stormwater entering the basin.
2276597 Primary Facility 2014-05-28 Incident- Response to Accident or Event No Violations Noted 5/26/2014: Tanya Montefusco (Inflection) reported a synthetic oil-based mud spill from the Flexrig 354 after encountering gas while drilling the Bush Hill Bennett 2 (081-21351). The displaced drill mud was sprayed across the well pad, onto the soil stockpile, and on some trees.
2421105 Primary Facility 2015-10-27 Routine/Complete Inspection No Violations Noted : Weather calm, cloudy, cool. Arrived @ 15:50. There were 4 wells spudded on the site, 3 production tanks, 1production unit, and a bank of 4 Baker tanks on containmentChaapel Mountain Bennett 3: Well head capped in empty cellar.
2427764 Primary Facility 2015-11-23 Routine/Complete Inspection No Violations Noted Weather sunny, cold and windy. Arrived @ 11:35. There were 4 wells spudded on the site, 3 production tanks, 1production unit, and a bank of 4 Baker tanks on containment.
2438463 Primary Facility 2016-01-04 Complaint Inspection No Violations Noted Cold sunny, snow on the ground. Arrived @ 13:20. There were 4 wells spudded on the site, 3 production tanks, 1production unit, and a bank of 4 Baker tanks on containment.

Chaapel Mountain Bennett 3: Well head capped in empty cellar. Annular vents were monitored for combustible gas using an Altair 5X meter calibrated on 01/04/2016. No gas was detected. There were no violations at this time. Left location @ 14:30.

2452087 Primary Facility 2016-02-18 Drilling/Alteration No Violations Noted Weather sunny, cold, patchy snow and ice on ground. Arrived @ 8:00. There was one well in production (BHB 2H), and 3 wells which were top-holed: BHB 1H – retrievable bridge plug @ 1700’, CMB 1H, CMB 2H.

Met with Brian Twigg (Inflection well-tender) and Ken Halk (Penn E&R). Plan was to blow down and measure flow rate on the intermediate casing for the Chaapel Mt. Bennett 1H and Chaapel Mt. Bennett 3H. Both top-holed wells have been shut-in for over 2 years. Inflection’s recent monitoring indicates that surface pressures exceed 1000 psi. Since blow-down was delayed, Ken Halk and I went to the Hamilton pad: left location 10:12, returned to Bennett @ 11:35, Both wells were being blown down when we returned. CMB 3H Pressure prior to blow-down 1200 psi. As Brian opened the 9-5/8” casing vent the well emitted gas and some mud and made a vigorous roaring sound that did not stabilize during the inspection. Previously on 2/9/15, the well took 2.5 hours to blow down from a starting pressure of 1200 psi. Equipment lim

Chaapel Mountain Bennett 3: Well head capped in empty cellar. At the time of my inspection, wireline crew was mobilized to open a 2” valve in a retrievable bridge plug at 1700’ on the Bush Hill Bennett 1H.

2479867 Primary Facility 2016-04-26 Routine/Complete Inspection No Violations Noted Arrived @ 16:00. Weather: sunny, humid. The BHB 1H was shut-in on 4/25/16. The two other top-holed wells: Chaapel Mountain Bennett 1H and Chaapel Mountain Bennett 3H have been shut-in, but pressure monitoring for these two wells also began on 4/25/16 prior to a pressure and flow monitoring study. BHB 1H: shut-in. Intermediate casing pressure: 1600 psi, (0-3000 psi analog gauge). BHB 2H in production. Chaapel Mountain Bennett 1H: Intermediate casing pressure: 1070 psi (0-3000 psi analog gauge). Chappel Mountain Bennett 3H: Intermediate casing pressure: 920 psi (0-3000 psi analog gauge).

Plan is to begin flowing the wells on 4/28/16 and begin measuring rates.

2492580 Primary Facility 2016-06-09 Drilling/Alteration No Violations Noted
2535032 Primary Facility 2016-11-02 Routine/Complete Inspection No Violations Noted Arrived @ 13:30, weather warm and sunny for this routine inspection with no personnel on site. This location was previously hit by historically heavy rain which damaged the access road

Chaapel Mountain Bennett 3H: This well was top-holed (01/2014). There are three annular vents: one, 1”-diameter 20” x 13-3/8” vent and two, 2” 13-3/8” x 9-5/8” vents. The vents are clearly labeled. No combustible gas was detected on any of the annuli. Left @ 14:10

2564375 Primary Facility 2017-02-22 Routine/Complete Inspection No Violations Noted
2570838 Primary Facility 2017-03-13 Administrative/File Review Violation(s) Noted Administrative Inspection Record created for the purpose of entering Enforcement Record for failure to plug an abandoned well.

VIOL closed 1/12/2018 due to permit mod being issued for this well as a result of VIOLS cited in IR #2570838 and ENF #351602 for failing to drill the well in the permitted surface hole location. Because the well has an active well permit, the VIOL for failure to plug an abandoned well was ACO.

2605625 Primary Facility 2017-06-20 Administrative/File Review Violation(s) Noted Administrative Inspection entered for the purpose of entering an Enforcement Record (NOV).

VIOLS closed (CRCTD) 1/12/2018 upon issuance of permit modification.

2818993 Primary Facility 2018-12-26 Routine/Complete Inspection No Violations Noted One well staged for production (2H). Three additional wells spudded, but not staged for production. These wells are recommended for in active status application with the Department.

One GPU within containment. No violations taken at this time.

Additionally, a fresh water impoundment is on location with maintaind free board.
