2236467 Primary Facility 2014-01-09 Routine/Partial Inspection No Violations Noted Dallas Morris 25 onsite waiting for conductor to be drilled. No Vio.
2274687 Primary Facility 2014-05-21 Complaint Inspection No Violations Noted On 5/21/2014 Ms. Renner and I inspected the site. Severe erosion was noted on the cut slope of the infiltration basin and at the inlet swale entering the basin. In addition, erosion was noted in the roadside swale above the basin. Based on the erosion I recommend re-evaluating the basin design itself in addition to possibly adding additional cross culverts to reduce the volume of stormwater entering the basin.
2276596 Primary Facility 2014-05-28 Incident- Response to Accident or Event No Violations Noted 5/26/2014: Tanya Montefusco (Inflection) reported a synthetic oil-based mud spill from the Flexrig 354 after encountering gas while drilling the Bush Hill Bennett 2 (081-21351). The displaced drill mud was sprayed across the well pad, onto the soil stockpile, and on some trees.
2421104 Primary Facility 2015-10-27 Routine/Complete Inspection No Violations Noted Weather calm, cloudy, cool. Arrived @ 15:50. There were 4 wells spudded on the site, 3 production tanks, 1production unit, and a bank of 4 Baker tanks on containment.Chaapel Mountain Bennett 1H: Well head capped, cellar water-filled. One large bubble was observed approximately every 16-20 seconds on the side of the well head closest to the BHB2. The bubbling was coming from the blind capping the casing head, according to Fidurko, who said it would be tightened up on 11/2/15.
2427713 Primary Facility 2015-11-23 Routine/Complete Inspection No Violations Noted Weather sunny, cold and windy. Arrived @ 11:35. There were 4 wells spudded on the site, 3 production tanks, 1production unit, and a bank of 4 Baker tanks on containment.

Chaapel Mountain Bennett 1H: Well head capped, cellar empty. Annular vent were monitored for combustible gas using an Altair 5X gas meter calibrated on 01/04/2016. No gas was detected. There were no violations at this time. Left location @ 14:30.

2452048 Primary Facility 2016-02-18 Drilling/Alteration No Violations Noted CMB 1H:

Pressure prior to blow-down 1250 psi. As Brian opened the 9-5/8” casing vent the well emitted gas and some mud and made a vigorous roaring sound that did not stabilize during the inspection. Previously on 2/9/15, the well took 3 hours to blow down from a starting pressure of 1250 psi. As Ken’s equipment could not measure flows > 20l/min (0.7 ft3 /min). The blow-down was terminated @ 12:42, approximately 1.5 hours. Left location @ 12:45.

2454733 Primary Facility 2016-03-01 Routine/Complete Inspection No Violations Noted Weather sunny, spring-like. Arrived @ 13:25. There were 4 wells spudded on the site, 3 production tanks, 1production unit, and a bank of 4 Baker tanks on containment. A water truck was leaving the site as I arrived.

Chaapel Mountain Bennett 1H: Water and ice in the cellar, no bubbling. This well was top-holed (01/2014). There are three annular vents: one, 1”-diameter 20” x 13-3/8” vent and two, 2” 13-3/8” x 9-5/8” vents. The vents are clearly labeled. They were monitored for combustible gas using an Altair 5X meter calibrated on 03/01/16. 20” x 13-3/8”: 0% methane 13-3/8” x 9-5/8”: 0% methane Left location @ 14:00. There were no violations associated with this inspection.

2472554 Primary Facility 2016-04-07 Routine/Complete Inspection No Violations Noted
2479855 Primary Facility 2016-04-26 Routine/Complete Inspection No Violations Noted Arrived @ 16:00. Weather: sunny, humid. The BHB 1H was shut-in on 4/25/16. The two other top-holed wells: Chaapel Mountain Bennett 1H and Chaapel Mountain Bennett 3H have been shut-in, but pressure monitoring for these two wells also began on 4/25/16 prior to a pressure and flow monitoring study. BHB 1H: shut-in. Intermediate casing pressure: 1600 psi, (0-3000 psi analog gauge). BHB 2H in production. Chaapel Mountain Bennett 1H: Intermediate casing pressure: 1070 psi (0-3000 psi analog gauge). Chappel Mountain Bennett 3H: Intermediate casing pressure: 920 psi (0-3000 psi analog gauge).

Plan is to begin flowing the wells on 4/28/16 and begin measuring rates.

2492579 Primary Facility 2016-06-09 Drilling/Alteration No Violations Noted Arrived @ 10:45. Weather: sunny, pleasant. Personnel on site: Brian Twigg (Inflection), Matt Caldwell (Moody). The purpose of this inspection was to check on the monitoring/flaring arrangements for the 3 top-holed wells on location: BHB 1H, CMB 1H, and CMB 3H, following receipt of Inflection’s update on Intermediate Casing Pressure management for these wells. At the time of inspection: BHB 1H was flaring, and CMB 1H and CMB 3H were shut-in, with intermediate hut-in pressures at 200 psi. Continuous pressure loggers were fitted to the intermediate casings of each top-holed well. Beginning 6/14 @ 6:00, the BHB 1H would also be shut-in. Pressures on the CMB 1H and CMB 3H would be monitored to determine whether a hydraulic connection between the BHB 1H and the CMB wells exists at depth. No violations were observed during this inspection. Left location @ 11:20.
2535035 Primary Facility 2016-11-02 Routine/Complete Inspection No Violations Noted Well top-holed, no gas on annuli.
