2381554 Primary Facility 2015-06-22 Incident- Response to Accident or Event No Violations Noted On 6/19/2015, Christopher Becker called to report 5-10 gallons of diesel based drilling mud was found around the cellar of the 14H well while excavating to install flow lines from the well heads to the production units. On 6/22/2015, I again talked to Mr. Chitwood and he explained that they had been working all weekend to delineate the release and stockpile impacted material. The extent of the release had expanded from the 14H cellar west/southwest near adjacent wells about 30'-50'. Flow lines and associated infrastructure were being installed and the excavation of the diesel mud release was located to the south of the wells. RETTEW representatives were delineating the last of the release and PID readings were about 100 ppm with more excavation planned. Violations cited to the 14H (081-21552).
2482686 Primary Facility 2016-05-06 Routine/Complete Inspection No Violations Noted I entered the location at 09:10 and began the inspection. At the time of the inspection, the John Good 7H (081-21452), 8H (081-21450), 9H (081-21451), and 14H (081-21552) were in production. The well pad area and access road are all stabilized and no erosion was noted. The well site has not been restored but the rock sumps have been installed based on the PCSM (Post Construction Stormwater Management) plan in the ESCGP. The revised restoration assessment indicates this site has an active well permit but currently the Departments records indicate the John Good 13H (081-21571) was reported Proposed But Never Materialized since the well was permitted on 2/17/2015 and was never drilled. The well pad remains at its original size and no well permits have been issued since 2015.

The Department considers a well site to be restored under Section 3216(c) of the 2012 Oil and Gas Act when the oil or gas owners or operators meet the following:

2568412 Primary Facility 2017-03-04 Routine/Complete Inspection No Violations Noted Inspection of SWN’s John Good (LU-09 Good West) pad in Jackson Twp., Lycoming County was conducted Friday, March 3rd 2017 at 10:40 am. The operator contacted me late afternoon yesterday to report bubbling inside the surface casing in the (13 3/8” x 9 5/8”) intermediate casing’s annulus on the John Good 14H well. At the time of inspection; the 14H had a production tree in place and was open to sales. The production tree had digital pressure gauges; (1) on the wing valve (592 PSI, production casing) and (1) on the line out to production (322 PSI, tubing). The backside of production casing (9 5/8” x 5 ½”) annulus was open on the wellhead, plumbed to surface where it was shut-in/isolated from line to sales/choke to condensate tank. There was no readable gauge on this annulus at time of inspection but a taylor plug valve inline to check pressure with a gauge probe.

The casinghead for the well is set on the 9 5/8” casing and the 13 3/8” surface casing was cut below grade in the cellar which was full of water. Loo

2681104 Primary Facility 2018-01-03 Routine/Complete Inspection No Violations Noted The access road from the Good pad and impoundment to the well pad itself was in good condition and well maintained. A few inches of snow was covering the entire site. On the well pad, four wells were in production. Sand separators remain installed at the production units and both (for each well) are in containment. No staining was visible around any of the production equipment.

The ESCGP permit has expired and Post Construction Stormwater Management (PCSM) has not yet been permitted or implemented. I recommend re-permitting the site and following the COA which indicates this site will be restored in 2020. No violations noted.
