I met with Ben Gamble (DCNR) at approximately 09:05 and we met with the company man (Bryan Billiot) and we walked the site. At the time of the inspection, Keane was onsite to complete several wells which were previously drilled to TD. The COP Tract 356 G 1039H well was being prepared for the first stage of the frac. The site containment was in good condition and foam berms were adequate. The containment berms were of different heights at several locations causing some confusion about the containment. I recommend evaluating the containment at the flowback tanks in the future. I also talked to Carl Wool from Keane concerning individual containments under equipment and general practices. No violations noted.

2744761 Primary Facility 2018-06-20 Routine/Complete Inspection No Violations Noted Site active, moving equipment. No company man on location. Containment appeared to be in good condition. Several spots on the liner had puddles that exhibited elevated conductivity (3.2- greater than 19.99 mS/cm), with no evidence of the same on the pad surface. I recommend that the containment continue to be vacced regularly.
2758532 Primary Facility 2018-07-30 Routine/Complete Inspection No Violations Noted Weather was 71° and raining at time of inspection. Met with Alta representative Charles Egli on location. Well was shut in at time of inspection. Well had recently been stimulated by hydraulic fracturing. Crews were on location welding and hooking up Gas production equipment. Well was locked out and tagged out. There was no tag on the well at time of inspection. I received a call recently from Rick Hall that new metal tags were in the process of being made and attached. There was one annulus vent that was plumbed to surface that was shut in at time of inspection. No violations noted at time of inspection.
2781606 Primary Facility 2018-09-26 Routine/Complete Inspection No Violations Noted Arrived on site for routine inspection of pads F & G. Pad F is currently empty but appears stable. Wells on pad G were somewhat recently altered/stimulated.

Several temporary flowback tanks were onsite at the time of inspection. The tanks were in good condition and in secondary containment.

It appears that wells 1039H, 1040H, 1041H are staged for production and 1037H, 1042H have been granted inactive status. There are 4 produced water tanks onsite and in secondary containment. There are also 4 GPUs onsite and in secondary containment.

There is an additional unlabeled storage tank not in secondary containment. There are also wells that appear to be missing proper tags (Photos #11, 12, 13, 14). The missing tags have been previously noted in inspection #2758529 on 7/30/18. Spoke to Rick Hall on 9/28/18; the tags are made and will be installed after the well heads are painted.

Overall all the site is well vegetated and appears stable. Secondary containment is in good condition except for one area on the east

2825998 Primary Facility 2019-01-10 Routine/Complete Inspection No Violations Noted On January 10, 2019, Department representative Brad Kline conducted a routine site inspection at ARD Operating , LLC. COP Tract 356 Pad F & G. No E&S issues were observed during the time of inspection. The site is stable and well vegetated meeting the 70% uniform, perennial, vegetative cover.

There are a total of 5 producing wells located on this well pad. All wells are tagged with API/well permit numbers. No bubbling within the cellars was observed. There are 5 GPU’s and 4 steel vertical production fluids tanks onsite. Production fluid tanks and flowback tanks were stationed in secondary containment.

Pad G has no wells drilled on it, but appears to be stable.

No violations noted.
