H&P Flex Rig #225 on location, currently running tubing in the Mac 201H well. I noted that the containment berms in the traffic areas were damaged in several places, mostly on the top of the berm; while I was on-site, repairs were taking place on other areas of the liner. I noted a very small (less than 1 gallon) puddle on the pad near the mud tanks that had an oily sheen; R.D. said he would have it cleaned up. Near the same area, I noticed that grounding wires with a protective plastic cover were run over the foam berm, crushing it down to only 1-2" high, and mentioned to R.D. that this was a pathway for liquids to escape. He told me that he would correct it. |

Containment appears to be adequate in theory but areas surrounded with foam berms appear most susceptible to overtopping in the event of a release. I recommend evaluating containment and rig mats on the next locations as discussed. No violations noted. |

A workover rig was on site doing a toe prep, and equipment was being brought onsite for the upcoming frac. Sitewide containment was new, but Rodney informed me that when the frac tanks had been brought onsite, setting them down had resulted in several small holes in the liner from the pins on the tanks. Crew was in the process of repairing these holes, and Rodney had begun setting heavy duty mats under the pins to avoid more damage. Some frac iron had been set up with wood blocks under it, and I mentioned to Rodney rhat the blocks could still rub and damage the liner when the iron was pressurized. I recommended rubber mats under the blocks. |

On 12/12/2017, a subcontractor reported (to the operator) a failure at the blender caused frac fluid to be released to the site containment. The following day, water was discovered ponded off containment near the northwest corner of the site. An investigation revealed a hole in containment about 10 feet from the northern edge of containment where it is believed the spilled frac fluid was able to leave containment. Mark Barbier (ARD) called at 16:04, to report the release which was confirmed about an hour before the call. I met with Mr. Barbier onsite at approximately 10:25 and we walked along the west side of the well pad to the northwest corner. Along the fill slope at the northwest corner of the pad, a small puddle was visible where ARD reported the frac fluid was discovered outside containment. Field conductivity was 7,140 umhos/cm but the temperature censor was under the acceptable range so sample 3440339 was collected at 11:00. Due to the nature of the release, no estimate |

A workover rig was actively drilling plugs on the site. Jake walked the site with me. Several holes were noted in the sitewide containment, and all water that I tested for conductance on the liner was over 19.99 mS/cm (the upper limit of my meter). No free water was present in the intercept trench on the pad, and the intercept trench on the NW slope contained only ice. Some free water (1-2 gallons) was present off containment where traffic was pulling on and off of the liner; conductance of this fluid was over 19.99 mS/cm. After the inspection, Jake and I discussed strategies to avoid releases in the future, including addressing containment issues. Outstanding violations exist for the release. |
