Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2636017 Primary Facility 2017-09-14 Drilling/Alteration No Violations Noted H&P 225 rigging up. Met with RD Mann, company man, and Dan Lockwood,'drilling consultant. Left @9:40. The result of this inspection is NOVIO.
2638397 Primary Facility 2017-09-21 Drilling/Alteration No Violations Noted Arrived @12:39. Weather: sunny, low humidity, light wind. H&P 225 on site on the Mac A well. Met with RD Mann, company man. Reviewed Schlumberger surface casing cement report -45 bbl return to surface. Rig at 750' while BOP test ongoing. Test plug stuck in the well head at the time of my inspection. Left @ 13:03. The result of this inspection is NOVIO.
2640605 Primary Facility 2017-09-27 Drilling/Alteration No Violations Noted Arrived @11:55. Weather: sunny, unseasonably hot. H&P 225 on site on the Mac A well. Met with RD Mann, company man. Reviewed Schlumberger intermediate casing cement report. IC shoe at 2502'. Rig at 4053'. Walked location, using front loaders to handle cuttings. Left @ 13:00. The result of this inspection is NOVIO.
2643215 Primary Facility 2017-10-03 Drilling/Alteration No Violations Noted Arrived @13:25. Weather: sunny, cool. H&P 225 on site on the Mac A well. Met with Billy Mitchell, company man. At the time of my last inspection the BOP test plug was stuck in the well head. It had not proved possible to get the plug out, had to remove and replace the well head. At thentime of this inspection, drilling was at 12,477' on SOBM. Had encountered big cuttings from 9700' while building the curve. Had to bring mud weight up to 13.4 ppg. Discussed cuttings bins - ARD has moved back to standard cuttings management. Left @ 13:00. The result of this inspection is NOVIO. Left at 14:00.
2647478 Primary Facility 2017-10-13 Routine/Complete Inspection No Violations Noted I met with R. D. Mann when I arrived on-site. H&P Flex Rig #225 on location, currently running tubing in the Mac 201H well. I noted that the containment berms in the traffic areas were damaged in several places, mostly on the top of the berm; while I was on-site, repairs were taking place on other areas of the liner. I noted a very small (less than 1 gallon) puddle on the pad near the mud tanks that had an oily sheen; R.D. said he would have it cleaned up. Near the same area, I noticed that grounding wires with a protective plastic cover were run over the foam berm, crushing it down to only 1-2" high, and mentioned to R.D. that this was a pathway for liquids to escape. He told me that he would correct it.
2650592 Primary Facility 2017-10-16 Routine/Complete Inspection No Violations Noted I entered the site and talked to R.D. Mann (Company Man). The H&P 225 was onsite preparing to move from the MAC north A-201H (081-21700) to the Alden Evans A-202H (081-21699). The BOP stack was still on the MAC north A-201H. Containment appears to be adequate in theory but areas surrounded with foam berms appear most susceptible to overtopping in the event of a release. I recommend evaluating containment and rig mats on the next locations as discussed. No violations noted.
2652604 Primary Facility 2017-10-25 Drilling/Alteration No Violations Noted Arrived at 13:50 with OGI Ulyana Trynovich. Met with company man RD Mann also on location were RickHall and Mark Barbier (Alta). H&P 225 onsite at 6575' on the Alden Evens A-202H. The getting ready to kick off. IC set at 1350', FIT to 16.5 ppg equivalent. Reviewed cement reports for production casing cement on Mac North A201H well, surface and IC for Alden Evens A- 202H. no violations were noter during this inspection.
2664343 Primary Facility 2017-11-27 Routine/Complete Inspection No Violations Noted Arrived at 10:25. Weather: sunny, cold. Wells and pad being prepared for stimulation at the time of my inspection. Met with Rodney Blanton, company man. Frac heads were on the two wells on site. Tanks were being staged and tank locations delineated. Left at 11:00. There were no violations at the time of my inspection.
2666706 Primary Facility 2017-11-30 Routine/Complete Inspection No Violations Noted I met with Rodney Blanton when I arrived. A workover rig was on site doing a toe prep, and equipment was being brought onsite for the upcoming frac. Sitewide containment was new, but Rodney informed me that when the frac tanks had been brought onsite, setting them down had resulted in several small holes in the liner from the pins on the tanks. Crew was in the process of repairing these holes, and Rodney had begun setting heavy duty mats under the pins to avoid more damage. Some frac iron had been set up with wood blocks under it, and I mentioned to Rodney rhat the blocks could still rub and damage the liner when the iron was pressurized. I recommended rubber mats under the blocks.
2673242 Primary Facility 2017-12-20 Drilling/Alteration No Violations Noted Arrived at 12:00. Weather: sunny, windy, unseasonably warm. Met with Dean Krausse ( company man, Alta). Keane fraccing on Stage 1 of the Alden Evens A-202H. Frac going well. Met Mark Barbier, who showed me the interception trench at the edge of the pad.
2674018 Primary Facility 2017-12-14 Incident- Response to Accident or Event No Violations Noted Spill response inspection.

On 12/12/2017, a subcontractor reported (to the operator) a failure at the blender caused frac fluid to be released to the site containment. The following day, water was discovered ponded off containment near the northwest corner of the site. An investigation revealed a hole in containment about 10 feet from the northern edge of containment where it is believed the spilled frac fluid was able to leave containment. Mark Barbier (ARD) called at 16:04, to report the release which was confirmed about an hour before the call. I met with Mr. Barbier onsite at approximately 10:25 and we walked along the west side of the well pad to the northwest corner. Along the fill slope at the northwest corner of the pad, a small puddle was visible where ARD reported the frac fluid was discovered outside containment. Field conductivity was 7,140 umhos/cm but the temperature censor was under the acceptable range so sample 3440339 was collected at 11:00. Due to the nature of the release, no estimate

2682466 Primary Facility 2018-01-11 Routine/Complete Inspection Outstanding Violations - Viols Req'd I met with ARD company man Jake Benton when I arrived. A workover rig was actively drilling plugs on the site. Jake walked the site with me. Several holes were noted in the sitewide containment, and all water that I tested for conductance on the liner was over 19.99 mS/cm (the upper limit of my meter). No free water was present in the intercept trench on the pad, and the intercept trench on the NW slope contained only ice. Some free water (1-2 gallons) was present off containment where traffic was pulling on and off of the liner; conductance of this fluid was over 19.99 mS/cm. After the inspection, Jake and I discussed strategies to avoid releases in the future, including addressing containment issues. Outstanding violations exist for the release.
2853716 Primary Facility 2019-03-14 Routine/Complete Inspection No Violations Noted Two wells staged for production. Two GPUs with steel skid containment. A Capstone turbine generator is on location and in operation. No leaks from production equipment. Temporary BMPs are recommended to be removed.
