2660037 Primary Facility 2017-11-09 Routine/Complete Inspection No Violations Noted Rig had not yet arrived on-site, but containment and rig mats were in place. Powdered, bagged chemicals were being transferred to a storage location on containment on the access road; some white powdered material was visible on the berm of the containment, as well as on the access road itself. The same white powdered substance was present on the rig mats near the site entrance. Containment appeared to be good, with rigid berms around the rig mats.
2665244 Primary Facility 2017-11-28 Routine/Complete Inspection No Violations Noted H&P 225 rig on location, running on the A203H well (2515' depth). I met with Bill Mitchell, ARD company man for the site. He informed me that they were preparing to run intermediate casing in the well. The sitewide containment was in fairly good condition, with some minor damage to the liner at the top of the berms, none of which appeared to be lowering the containment volume. Some berms had been pushed loose from the chemical containment area; Bill told me he would repair it. The cuttings backyard was clean and well maintained.
2682889 Primary Facility 2018-01-11 Routine/Complete Inspection No Violations Noted I met with RD Mann when I arrived. Currently drilling the 202 well at 18,796' with approximately 2000' to go. Housekeeping issues were widespread. The rig mats in the cuttings backyard had a mix of cuttings and sawdust or other wood drying product all over, and a small amount had been tracked off of containment. Near this area, a small puddle (less than 1 gallon) of an oily product was present on the ice off of containment; a vac truck was immediately directed to the location. On containment near the centrifuge and mud pits, the entire surface of the rig mats had a skin of oil based drill mud on them, and a pile of drill mud (2' x 2', 1' high) was next to the mud pits. The vac truck was directed to this area as well. In the chemical containment, 2 totes were hanging partially off of containment, and several piles of powdered product were present. I recommend that a more rigorous housekeeping program be enforced for this site and future drilling activities.
2725613 Primary Facility 2018-05-01 Routine/Complete Inspection No Violations Noted Frac was active when I arrived. I met Rick Hall, Steve Barbier, and Marek Romatowski of ARD. Marek walked the site with me. Containments and berms were mostly in excellent condition. Secondary containment was deployed under all trucks on the site. In the southwest corner of the sitewide containment, 10-20 gallons of water had accumulated in a low spot in the containment that showed elevated conductance (17.1 mS/cm); the pad immediately next to this area was bone dry. Marek directed a vac truck to the area. Overall condition of the site was good.
2725715 Primary Facility 2018-04-18 Routine/Complete Inspection No Violations Noted Routine inspection; 4/18/2018.

I entered the site and met with the Company Man, Merek Romatowski. At the time of the inspection, active completions operations were ongoing (stage 60 of 64) on the Brooks Family A-202H. Freshwater was being pumped from the Mirto impoundment and a portable tank on the nearby Vargo Compressor Station. Field conductivity was 120 umhos/cm in containment for produced water. Some friction reducer was noticeable in containment near the blender. Dust from the sand moving equipment was visible and blowing east toward the middle of the site (see photo 2). I recommended to the Company Man that they continue to vacuum water from containments (vac truck was onsite working) and look into minimizing silica dust onsite. No violations noted.

2731747 Primary Facility 2018-05-11 Complaint Inspection No Violations Noted Complaint response inspection; 5/11/2018.

I entered the site and met with the Company Man, Dean Krouse. The purpose of the inspection was to respond to a noise complaint from a nearby landowner. Although the Department does not have specific regulations related to noise, my inspection was merely to explore the concerns and discuss my observations with the complainant. I used the Decibel X app (iOS) to measure and document my findings.

At the time of the inspection, the BJ Services Frac crew was between stages, so the noise on the site was minimal. At approximately 13:45, pumping began on the next stage. The noise began to increase as additional pumps/engines came online. At approximately 14:30 the noise level appeared to stabilize at the highest level detected. At 14:34, I went up to the site entrance off Rose Valley Road. At the entrance approximately 75 decibels were detected. 2763070 Primary Facility 2018-08-07 Routine/Complete Inspection No Violations Noted Notice of Termination Inspection; 8/7/2018.

At the time of the inspection, 3 wells were in production. Produced water was being pumped from the production tanks to a battery of frac tanks in containment. Field conductivity in the containment was between 200-300,umhos/cm. Post Construction Stormwater Management (PCSM) is accomplished with the use of a vegetative filter strip which remains undisturbed. Overall, the site fits the Site Restoration plans and can be considered restored. I recommend approving the Notice of Termination and I recommend the operator submit the Well Site Restoration Report as soon as possible. No violations noted.
