2818978 Primary Facility 2018-12-18 Routine/Complete Inspection No Violations Noted A routine inspection of this conventional gas well was conducted December 18, 2018. The purpose of this inspection was to determine the status of the well, in addition to investigate the well as a potential abandoned well. Department records indicate the well was originally drilled as an Oriskany well, but was partially plugged back for a shallow completion shortly after.

At the time of inspection, the entrance to the access road was gated and locked. I walked approximately 1/4 mile uphill to the well location. The access road was grass-covered with a few small trees, and in good general condition. The well location was open and clear with the exception of a few small trees.

The well has a conventional wellhead; 8 5/8" surface casing, 8 5/8" x 4 1/2" swedge, casing head w/ side outlets (4 1/2" casing partially recovered after the bottom hole was plugged), 1" tubing string in well sealed with hammer union packoff assembly. The well is shut in and not tied into equipment or pipeline. The well was not equipp
