2412160 Primary Facility 2015-09-19 Compliance Evaluation Outstanding Violations - No Viols Req'd The Department inspected the Carrier Well No. 1 on 9/18/15 @ 1000 hours in response to receipt of an email dated 9/18/15 showing photographs of the encapsulation of the drill cuttings pit. The area where the pit had been located appears to be filled in. No seed was observed on the surface. Some straw/hay was observed on the surface. The area does not appear to have been graded to achieve positive drainage. A berm of topsoil that appears to have been placed around the perimeter when the pit was dug, was not used to regrade the area and is still visible. Please see the attached pictures.
2469458 Primary Facility 2016-03-24 Follow-up Inspection Outstanding Violations - No Viols Req'd The Department of Environmental Protection (DEP) inspected the Allegheny Enterprises, Inc. (AEI) Carrier Well No.1 (API#105-21724) on 3/24/2016 @ 1100 hours to follow-up on outstanding violations (NOVs) cited during a 6/24/2015 inspection (inspection ID 2382498). The weather was 60° and sunny. At the time of this inspection, a loud hissing noise and strong odor were observed coming from the Carrier Well No.1 well head. The Altair 5x combustible gas meter was used to screen the well head. The over range alarm for combustible gas sounded. I immediately distanced myself from the area surrounding the well head. This issue has been referred to the Oil and Gas inpsector covering this area.The site is still in violation of section 58 Pa.C.S. §3216(a) for not restoring the well site within 9 months after completion of drilling a well. No new violations are sited at this time. I departed the site @ 1215 hours.
2514244 Primary Facility 2016-08-26 Follow-up Inspection Outstanding Violations - No Viols Req'd The Department of Environmental Protection (DEP) inspected the Allegheny Enterprises, Inc. (AEI) Carrier Well No.1 (API#105-21724) on 826/2016 @ 1300 hours to follow-up on outstanding violations (NOVs) cited during a 6/24/2015 inspection. The weather was 75° and sunny.

At the time of this inspection, a loud hissing noise and strong odor were observed coming from the Carrier Well No.1 well head. The Altair 5x combustible gas meter was used to screen the well head. The over range alarm for combustible gas sounded. The Altair 5X readings were obtained from the thread around the base of the well head. I immediately distanced myself from the area surrounding the well head. This issue has been referred to the Oil and Gas inspector covering this area. The area where the pit was located has been stabilized by volunteer vegetation. The berm of topsoil surrounding the pit area is still in place. The area does not appear to have been graded to achieve positive drainage.

2613025 Primary Facility 2017-07-07 Follow-up Inspection Outstanding Violations - Viols Req'd On 7/7/17 at 0902 hrs I conducted a follow up inspection at the Carrier 1 well site. It was partly sunny and 71 degrees F. The landowner, Ron Carrier, accompanied me to the site.

Violations were cited against the Carrier 1 well on 6/24/15 for an open drill cuttings pit. The pit has since been reclaimed; however, during reclamation, the surface of the encapsulated pit was not graded properly to encourage drainage. The landowner stated that the well operator conducted a site visit approximately a month ago and stated that the ground covering the pit would be regraded to improve drainage.

A production tank still remains on site. The tank should be removed if the operator does not intend to bring the well into production in the near term.

The site has been restored and is permanently stabilized as per 25 Pa Code Section 102.22(a). A Well Site Restoration report should be submitted as per 25 Pa Code Section 78.65(3).

2627027 Primary Facility 2017-08-17 Compliance Evaluation No Violations Noted On 8/17/17 at 1400 hrs I conducted an inspection at the Carrier 1 well site. It was mostly sunny and 82 degrees F. The landowner, Ron Carrier, accompanied me to the site. The Carrier 1 well was recently transferred from Allegany Enterprises to Roulette Oil and Gas.

Violations were cited against the Carrier 1 well on 6/24/15 for an open drill cuttings pit. The pit has since been reclaimed; however, during reclamation, the surface of the encapsulated pit was not graded properly to encourage drainage. Since the last follow up inspection on 7/7/17, the pit surface has been regraded, fertilized and seeded and mulched. Vegetation has germinated and is becoming established.

In addition, the well has been connected to the pipeline and the production tank moved into place in anticipation of production. The operator submitted a well site restoration report, which was received on 8/14/17. The report was acknowledged on 8/18/17.
