2014343 Primary Facility 2011-10-25 Routine/Complete Inspection No Violations Noted On Oct. 25, 2011 a routine inspection was conducted at the Triana Guardian Pad B well site. The site ID sign, E&S plan, E&S controls and perimeter of the site were observed. The access road and associated controls were installed and well maintained. The last self-inspection was conducted on 10/21/11. The slopes were stabilized. They had been hydro-seeded with some grass presently growing. Two conductors had been set and one well drilled. Bubbling was evident around the casing of the drilled well. Mike Panettieri, Oil and Gas Inspector was notified. No violations were cited during this inspection. - MRN

2185813 Primary Facility 2013-07-18 Routine/Partial Inspection No Violations Noted An inspection of the Triana Guardian Pad B in Hebron Township, Potter County was conducted on July 18, 2013. The site is stabilized, no puddles screened exhibited elevated conductance and no issues were noted. The status of the 2H well(105-21735) is Operator Reported Not Drilled (ORND), the 4H(105-21736) well is not drilled, but the permit is active and 6H (105-21737) is drilled and the permit is active. The mailbox at this site could not be accessed. No violations were cited during this inspection.
2260220 Primary Facility 2014-04-07 Routine/Partial Inspection No Violations Noted An inspection of the Triana Guardian Pad B in Hebron Township, Potter County was conducted on April 8, 2014. The status of the 2H well(105-21735) is Operator Reported Not Drilled (ORND), the 4H(105-21736) well is not drilled, but the permit is active and 6H (105-21737) is drilled to total depth but has not been completed. The Erosion and Sedimentation Control permit expires on October 31, 2015. The last inspection conducted on behalf of the operator was on April 3, 2014; no issues were noted. The perimeter erosion controls were installed and appear functional. Very little topsoil is present at the site; it is mostly rock. The site was sparsely vegetated, but stabilized. No active erosion was noted and no sediment transport was observed. The Department is requesting Triana Energy to submit a plan outlining their restoration schedule for this site as well as other sites operated by Triana.
2462914 Primary Facility 2016-03-04 Routine/Complete Inspection No Violations Noted Routine inspection. The 6H well has been drilled and is on inactive status (9/7/12). The site has not been restored. A request to extend the restoration requirements was approved on 1/16/15. A slump was oberved on the north fill slope. Requested that the slump be repaired and stabilized when conditions allow. TLT
2647338 Primary Facility 2017-10-12 Routine/Complete Inspection No Violations Noted On 10/12/17 at 0935 hrs I conducted an inspection of the Guardian Pad B well site. It was overcast and 56 degrees F. Also present during the inspection were Jim Reynolds and Pat Howard, Roulette Oil and Gas, and Jared Hurlbert, HD Engineering.

The 6H well has been drilled, however, not stimulated. The well is on inactive status.

The site consists of the well pad, access road and E&S controls. The site is permanently stabilized. It was originally permitted by an ESCGP-1 on 11/1/10; an ESCGP-2 was issued on 10/1/15. Interim restoration of the site has not been completed. Mr. Hurlbert will be evaluating the site to determine whether additional post-construction stormwater management controls are necessary. The last routine E&S inspection was on 10/12/17.

No violations were cited during this inspection. I concluded the inspection at 1230 hrs. TLT
