2597481 Primary Facility 2017-05-24 Routine/Partial Inspection No Violations Noted On 5/24/17 at 0953 hrs I attended a preconstruction meeting for the Sweden Valley 105 well site. The following people were present: JKLM Energy representatives, Jeff Long, John Manuel and Jason Logsdon; J.L. Watts representative, John Watts; Hillsdale representative, Rick Frantz; and New Growth Resources representatives, Jim Fowler and Rick Young. Penn E&R developed the Erosion and Sediment (E&S) control plan and J.L Watts is the construction contractor.

We reviewed the E&S drawing. Site drainage is to Nelson Run, which is designated as a High Quality stream. Total disturbed area, according to the E&S plan, will be 10.82 acres. The site will consist of an access road, a well pad and two sediment traps, which will be located beyond the northwest side of the pad. Two wetlands, both under one acre in size, are located near the proposed pad. One wetland is adjacent to the south pad corner; the E&S plan calls for high visibility fencing along the portion of the wetland that is immediately adjacent to the pad. T

2617116 Primary Facility 2017-07-18 Routine/Partial Inspection No Violations Noted On 7/18/17 at 1323 hrs I conducted a partial inspection of the Sweden Valley 105 well site. John Watts, J.L. Watts representative, and JKLM Energy representatives, Jeff Long and John Manuel, were present. During the inspection, I observed the access road construction and reviewed the erosion and sediment (E&S) documents maintained at the site. It was mostly sunny and 82 degrees F.

Clearing for the access road and well pad has been completed and earthwork began on 7/13/17. During my inspection, the contractor was constructing the access road and was approximately at station 3+50. Site construction is proceeding as per the construction sequence in the E&S plan.

Wood chip filter berms have been implemented in lieu of silt sock; this was previously discussed during the preconstruction meeting. As a reminder, mulch should be blown along the upgradient side of the berms as per the specifications in the E&S manual. The rock-lined ditch to the left of the access road, near the site entrance, has been installed.

2633560 Primary Facility 2017-09-06 Preoperation Inspection No Violations Noted On 9/6/17 at 1113 hrs I conducted a preoperational inspection at the Sweden Valley 105 well site. JKLM representative, Jeff Long, and J.L. Watts representative, John Watts, were present. It was mostly cloudy and 63 degrees F.

I observed the site and the routine Erosion and Sediment (E&S) control inspection log. Site construction is complete. E&S controls are installed and disturbed areas have been stabilized with seed and mulch or hydroseeded; germination is uniform and vegetation is becoming established. The well pad has been soil-cemented and stabilized with rock.

Cellars have not yet been installed. The pad is currently being used to stage limestone for improvement work on the Billy Lewis road. In addition, Hillsdale, the pipeline contractor for the Sweden Valley Connector pipeline, is using the pad to stage materials and equipment.

The Sweden Valley 105 7V, 1HU, 2HU, 3HU and 5HU well permits have been issued.

2676713 Primary Facility 2017-12-28 Routine/Complete Inspection No Violations Noted An inspection of the JKLM Sweden Valley Well Pad 105 was conducted 12/28/2017. Present for the inspection were myself and Jason Logsdon, JKLM pipeline Supervisor. The weather was 8 degrees and partly sunny.

The pad was constructed during the summer/fall of 2017. Cellars have not yet been installed. Hillsdale, the pipeline contractor for the Sweden Valley Connector pipeline, is using the pad to stage materials and equipment.

The Sweden Valley 105 7V, 1HU, 2HU, 3HU and 5HU well permits have been issued. No violations are cited during this inspection. I departed the location at 1500 hrs.
