2811628 Primary Facility 2018-12-06 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) conducted a routine inspection of the JKLM-Woodcock Creek Well pad 125 located in Allegany Twp, Potter Co. on 12/6/2018 @1145 hours. The weather was 30 degrees, overcast, winds SW @12mph. There was ~6" of snow observed on the ground.

JKLM has submitted a major modification to the ESCGP to expand the LOD of the topsoil stockpile area. The earth moving contractor JL Watts discovered a larger volume of topsoil covering the permitted site than what was originally calculated. The proposed modification would add 1.59 acres to accommodate approximately 40,000 cubic yards of material. Details were discussed onsite with John Watts, contractor.

As of today's inspection the access road has been rough graded to the pad area. Equipment was working on grubbing the pad area, placing the topsoil in within the original LOD of the stockpile area. Wetlands appear to be flagged with orange fencing.

The last E&S inspection was conducted on 12/3/2018, with no issues noted. No v

The Department of Environmental Protection (DEP) conducted a PREOP inspection of the JKLM Woodcock Creek Well Pad 125, located in Allegany Twp, Potter Co. on 2/5/2018 @1000 hours. Present for the inspection were Jeff Long, JKLM; Jamie Spear, JKLM and John Watts, JL Watts Contractor. No activity was observed onsite.

The site consists of ~4200' of access road, a 350' x 500' well pad, two sediment traps, and a topsoil stockpile. As of today's inspection the access road and rock-lined channels were observed installed. Soil cement and cap rock has not been completed on the well pad. The out-slope of the well pad was observed stabilized with ECB matting, with TRM matting in the perimeter channels.

The perforated drains around the perimeter of the well pad were discharging into the corner PVC pipe that outlets into the sediment traps. Both sediment traps were observed holding water and discharging due to snow melt. No violations were noted at this time. JKLM plans to spud and dril
