1871496 | Primary Facility | 2010-03-22 | Drilling/Alteration | No Violations Noted | According to well records, permit was granted on 10/30/10. At the time of inspection, no E&S and IW issues. Pit aligned with 20 mil plastic aligned drilling pit and frac pit aligned also and security fence. No free board issues with pit. Halliburton on-site to perform wire line study. EQT contracted Highland Drilling. Current depth was at 4224 ft. Well constructed with conductor pipe (20¿ to 39 ft) and surface pipe (13 5/8¿ to 547 ft). Class A cement used at 500 sks and basket set at 94¿ and 514¿ with 12 bbl return. According to records, plug set at 3:45 am and started at 11; 45 am. EQT/Highland waited the 8 hrs after performing the cementing of the surface pipe. |
1872001 | Primary Facility | 2010-03-12 | Routine/Complete Inspection | No Violations Noted | E&S plan and ESCGP-1 located onsite during site inspection. Site inspected along woth WQS Supervisor Ed Ritzer and Jeff Kerr of EQT. The inspection was to evaluate the erosion control changes that would be necceasry due to the reduction in size of the site. It was agreed that a sediment trap would still be neccesary, but that it could be significantly reduced in size. Mr. Kerr stated that this work will be undertaken over the weekend prior to the commencement of drilling. |
1872005 | Primary Facility | 2010-03-18 | Routine/Complete Inspection | No Violations Noted | Rig located onsite and drilling was ongoing at the time of the inspection. Below grade pit was located onsite and had more than the required two feet of lined freeboard. Observed no evidence of accelerated erosion & sedimentation on the site or access road at the time of the inspection. Noted that the ditch lined with hay matting alongside the access road did not directly connect to the sediment trap and that the ditches flowing into the trap had not been temporarliy stabilized with hay matting or some other erosion control measure. These e&s measures should be implemented to insure additional generation of sediment in the ditches is minimized. Sediment trap installed and appeared to be functioning properly. |
1873424 | Primary Facility | 2010-03-25 | Drilling/Alteration | No Violations Noted | According to well records, permit was granted on 10/30/10. At the time of inspection, no E&S and IW issues. Pit aligned with 20 mil plastic aligned drilling pit and frac pit aligned also and security fence. No free board issues with pit. EQT contracted Highland Drilling. Current depth was at 5909 ft. 13 3/8¿ pipe to 556¿ with 500 sks. 9 5/8¿ to 4174 ft with 911 lead (saporlite) and 133 sks tail (class A). Cemented up to 360 ft. Highland is in the process of lowering the drilling pit fluid to frac tanks located on-site. Noted that socks located around site are installed improperly and should be staked in the center of socks. EQT/Highland waited the 8 hrs after performing the cementing of the surface pipe. |
1884505 | Primary Facility | 2010-05-12 | Routine/Complete Inspection | No Violations Noted | At the time of inspection, well was drilled not yet in production. Proper signage and E&S plan onsite. Access road was stable. Pit had been removed. Filter fence and filter sox were properly installed below gradient of well pad and successfully functioning. Sediment trap also appeared to be functioning properly. Site was reseeded and mulched. Recommended that operator regrade prior to final restoration. Also recommended that operator reinforce filter fence located below sediment trap that appeared loose, as well as add additional rock or hay matting to reinforce drainage area below sediment trap. No IW or E&S issues. |
1969650 | Primary Facility | 2011-04-29 | Routine/Complete Inspection | No Violations Noted | Well drilled not producing. Site Id sign, ES plan, ESCGP-1 onsite. Access road stable. Well completed 04/04/2010. Well site surrounded by fence and gate. Site has been seeded/mulched w/>70% observed growth surround well pad, but noted that erosion control blankets used on slope immediately surrounding well head have fallen& do not appear to exhibit >70% vegetative (uniform) growth, although pad is adequately stabilized with rock. Recommend operator reseed & mulch to ensure uniform vegetative growth capable of resisting erosion is sustained. Although site has been seeded/mulched it does not appear that proper grade is achieved, as minor ES concerns were noted along slope below well site. Noaccelerated erosion or sedimentation issues were noted in relation to well site or access road at time of inspection. Although operator should continue to monitor bmp¿s, ie. sediment ditches and traps. Follow up inspection will be conducted to ensure allareas are properly stabilized to permanently reduce the potential for possible ES concerns. |
2005607 | Primary Facility | 2011-09-22 | Routine/Complete Inspection | No Violations Noted | Well was completed on April 4, 2010 to a depth of 8955 ft. According to EQT Production Manager, Doug Fry, the YMCA well will be plugged in the fall of 2011. Plugging plan will be forwarded to inspector for review. Well head had a pressure of 1800 psi. A surface stain was noted at the site near the former site pit. DEP inspector contacted Lisa Reaves from EQT and DEP water quality personnel, Marc Maestra and requested additional investigation of the stain area. At the time of the inspection, no E&S issues at the site. The site ID sign was visible at the access road. |
2009466 | Primary Facility | 2011-10-04 | Routine/Complete Inspection | In Compliance with Policy | : Site visit was in response to staining on the pad. April Weiland (PA DEP), Lisa Reaves (EQT) and Chris (EQT) were present during visit. |
2020384 | Primary Facility | 2011-11-09 | Plugging(Includes Plugged/Mined Through) | No Violations Noted | Well was completed on April 4, 2010 to a depth of 8955 ft. According to EQT Production Manager, Doug Fry, the YMCA well will be plugged in the November 2011. Plugging plan was submitted to inspector for review. Well head had a pressure of 1800 psi. Surface stain was noted at the site near the former site pit. At the time of the inspection, no E&S issues at the site. The site ID sign was visible at the access road. Hydrocarbon Drilling drill truck present. No site activity y occurred during the time of the inspection |
2270780 | Primary Facility | 2014-05-15 | Administrative/File Review | No Violations Noted | Admin Inspection written to update previous Inspection ID #2009466 results from Pending to ICP.
