Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1830143 Primary Facility 2009-09-18 Drilling/Alteration No Violations Noted An existing road was used to access the well site. The Site ID sign and E/S Plan were present at the entrance of the access road. The rig on site is Nomac 246. At the time of inspection the Company Man indicated they were drilling an 8-3/4" hole to a projected TD of approximately 12,700'. Drilling depth at the time of inspection was at 9135' in the Marcellus formation. Once TD is reached, 5-1/4" production casing will be set and cemented.
1856677 Primary Facility 2010-01-12 Routine/Complete Inspection No Violations Noted The well installation is complete and the well is online. A well tag with the well name and DEP permit number has been installed on the well head. Pipeline construction activities were occurring at the site during inspection.
1993298 Primary Facility 2011-08-01 Follow-up Inspection Violation(s) Noted Department records indicate that Chesapeake has failed to restore the site within nine (9) months after completion of drilling. Department records indicate the drilling completion date for the White 5H was October 8, 2009. This constitutes a violation of Section 206(c) of the Oil and Gas Act, 58 P.S. §601.206(c). The Department has revealed the evidence of fluids being treated and stored at the White 5H. The drilling completion date was October 8, 2009. The site operations constitute an unpermitted residual waste processing facility. The violations in reference to this are Section 301 of the PA Solid Waste Management Act, 35 PS § 6018.301 and Section 6018.610, item numbers 2,4 and 9 of the PA solid Waste Management Act, 35 P.S. § 6018.610.
