1860507 | Primary Facility | 2010-01-22 | Drilling/Alteration | No Violations Noted | At time of inspection I met with Company Man Jesus Villasana. Depth of drilling was at 7982¿. BOP¿s were in place and tested. Tophole down to kickoff point was predrilled. Pioneer is the drilling contractor on site. During the site walk I observed a large tear in the reserve pit liner which limited the available freeboard to approximately 3¿. I notified the Company man of the tear. I also observed a portion of the liner located underneath the boiler unit was melted through, exposing the bare ground below. I showed this to the Company Man and discussed installing some type of protective shield underneath the pipes to prevent future melting of the liner in this area. It was also noted that the crew was cleaning up a substantial amount of drilling mud from a hose failure located near the boiler unit. Based on my inspection, the drilling mud was contained on plastic, and not spilled onto the pad. The crew was in the process of cleaning the mud with squeegees and a vac-truck. |

2496280 | Primary Facility | 2016-06-27 | Routine/Complete Inspection | No Violations Noted | At time of inspection, arrived on site at 10:35a.m., the cellar was about a quarter full of water with no bubbling noticed, the (9x5) vent pipe was open and venting to a tank, there was 576psi on the production casing. I took a walk around the location and everything looked OK.
I left the site at 11:10a.m. |

2705254 | Primary Facility | 2018-03-08 | Routine/Complete Inspection | No Violations Noted | Arrived at location at 08 30. Weather: Overcast, cold, ~ 25 degs. (2) wells were present on location, the Shields, G. 1V and 5H. Both wells were producing at time of inspection. Both wells production pressures were being monitored by remote monitors sending data to Total Flow Control Units "TFCU"s with LED readouts located by production units. Each well had (1) annular vent plumbed to a shared poly vent tank. The 1V wells annular vent was labeled (9 5/8" x 4 1/2") and the 5H wells annular vent was labeled (9 5/8" x 5 1/2"). Both wells annular vents were open and venting to vent tank. A gas check performed at the shared vent tank opening showed: 100% LEL, 100% CH4. Gas check performed 3' away from vent tank showed 0% LEL, 0% CH4.
- Review of well history showed that previous 72 hr. pressure build up tests "PBUT" had been performed on these wells on (10/29 - 11/1/11) and (12/28 - 12/31/11). The initial PBUT showed the 1V, (9x4) press. build to 550 psi. and the 5H, (9x5) built to 8 psi. Subsequently, Cabot perf |
2709962 | Primary Facility | 2018-03-19 | Routine/Complete Inspection | No Violations Noted | Arrived at location at 08:45. Weather: Clear, cold, ~ 30 degs. (2) wells were present on location, the Shields, G. 1V and 5H. Both wells were producing at time of inspection. Onsite to witness start of the 72hr. press. build up test "PBUT" being performed on both wells surface x production casing annuli. Upon arrival met with Cabot O&G Reps., Adam Vogt, Aaron Onks, Brody Webster and Steve Catalfamo onsite. Both wells surface x production casing annuli were equipped with pressure logging units tied into Cabot's cygnet monitoring system. Adam confirmed communication with loggers through cynet system. These annuli were shut in at 09:00. We will return to site on Thursday, March 22 at 09:00 for conclusion of the 72 hr. PBUT.
Wells pressures were taken off LED readouts mounted at production units. (Prod. Casing Press. = CP, Static Press. = SP, Differential Press. = DP.) Shields, G. 1V: CP = 466.8 psi., SP = 491.4 psi., DP = 9.0 psi. Shields, G. 5H: CP = 490.7 psi., SP = 493.8 psi., DP = 61.8 psi. No obvious c |
2709998 | Primary Facility | 2018-03-22 | Routine/Complete Inspection | No Violations Noted | Arrived at location at 09:00. Weather: Clear, cold, ~ 30 degs. Onsite for the conclusion of the 72hr. press. build up test "PBUT" being performed on both wells surface x production casing annuli. The PBUT was started on 3/19/18 at 09:00. Upon arrival met with Cabot O&G Reps., Adam Vogt & Aaron Onks and (3) Selman & Assoc. Reps. Both wells surface x production casing annuli were equipped with pressure logging units tied into Cabot's cygnet monitoring system. The PBUT ending pressures were:
- 1V: (9 5/8"x 4 1/2") = 212 psi. - 5H: (9 5/8"x 5 1/2") = 42 psi. Gas checks performed by the Dept. and Selman showed: 100% CH4. The Dept. and Selman collected gas samples off both these annuli at approx. 09:30. Both wells annuli were bleed down and opened back up to poly vent tank. Post PBUT, water samples were collected at area resident by Eric Rooney, D.E.P. Geologist and Moody & Associates prior to bleeding pressures off of gas wells. - Review of well record for the 5H well showed the 5 1/2" prod. casing cement top wa |
