Filter Unit and Pump were provided by Rain for Rent. Met with Brady Stark and Kenneth Gumare of Rain for Rent. Filtration process started on 8/20/11 and thus far 12 flowback influent tanks have been completley filtered. Endless Mt. Energy Systems was in the process of vacuuming rainwater inside containment area where filter and pump are housed. Vaccuum trucks labeled fresh water and fresh water only were hauling filtered flowback effleunt off site for re-use. PPC plan and OG-71 approval form was in the mailbox on site access road. OG-71 was approved and issued on 8/19/11.

The (9x5) vent pipe was open and set up with a Taylor plug, there was 211psi on the production casing and 203psi on the production line. I took a walk around the location and everything looked OK.

Floyd Parrish of SWN was on site during my inspection. I left the site at 14:10p.m.

2593773 Primary Facility 2017-05-10 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:45p.m.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge. There was 132psi on the production casing and 100psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:20p.m.

2700563 Primary Facility 2018-03-01 Routine/Complete Inspection Pending At time of inspection.

The well was in production. The cellar was full of gravel and there was gas detected at 100% LEL and 20% methane at the perimeter of the cellar. I contacted Steve Moyer of SWN to suggest the cellar be cleaned out to determine where the gas was coming from. I took a walk around the location and there were no other obvious concerns noticed.

2727430 Primary Facility 2018-05-08 Routine/Complete Inspection No Violations Noted At time of inspection.

This inspection is a follow up inspection because gas was detected at the perimeter of the cellar during a routine inspection on 3/1/18. I met with SWN production foreman Cody Stanton. SWN had the gravel cleaned out of the cellar to determine where the gas was coming from. There was water between the (20x13) and (13x9) annuli and there was no bubbling noticed. Cody sprayed soapy water on the connections, we noticed bubbling where a nipple screws into a ball valve at the (9x5) section of the well head. Cody said they will remove all the connections and put bull plugs in all but one port. The last port will be plumbed to the surface and set up with a Taylor plug.

2800033 Primary Facility 2018-11-07 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water 1.5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
