2469235 Primary Facility 2016-03-30 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:25 p.m., the cellar was about a quarter full of water with no bubbling noticed, the (20x13) vent pipe was open and venting to a tank, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 621psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 13:50 p.m.

2852046 Primary Facility 2019-03-12 Routine/Complete Inspection No Violations Noted I inspected the Post pad on March 12, 2019. It was about 33 degrees F and flurrying. The sky was overcast.

The access road was short, mostly dry, and stable. The well pad was well graded and had some leftover snow patches. It did not have any areas of instability noted.

The pad appears to have recently been reworked, with all slopes well stabilized using matting, mulch or a combination of both. There were permanent PCSMs visible and all areas were stabilized. Untouched areas of the pad had adequate vegetation and there were no areas of concern noted.

The wells were clearly labeled.

There were no noted violations.
